目的為1例老年譫妄患者制定護理方案，檢索當前最佳證據，使譫妄癥狀消失，促進患者早日康復。方法 2015年4月13日收治1例老年患者，入院診斷：①肺部感染，Ⅰ型呼吸衰竭；②原發性高血壓3級，極高危；③意識障礙原因待查；④低蛋白血癥。患者有原因不明的意識障礙，護士全面評估病情后，確定患者存在譫妄。針對患者的譫妄問題，檢索數據庫獲得循證醫學證據，制定最佳護理方案，住院始末均全面實施該方案，評價護理結局。結果檢索納入循證指南2篇，系統評價4篇，在對老年譫妄患者進行護理時，針對譫妄的危險因素提前進行預防，采用多學科合作的非藥物干預手段，可以使老年患者的譫妄癥狀消失，改善護理質量。結論通過循證方法護理老年譫妄患者，更具有科學性和有效性。
