目的總結關節鏡下改良彈性固定 Latarjet 術治療 1 例慢性鎖定性肩關節前脫位的經驗。方法 2016 年 7 月收治 1 例因摔傷致右肩疼痛、畸形、活動受限 8 周的 49 歲男性患者。傷后曾于外院行肩關節復位，未成功。術前美國肩肘外科協會（ASES）評分 22 分，肩關節功能 Constant-Murley 評分 37 分，疼痛視覺模擬評分（VAS）8 分。影像學檢查示右肩關節鎖定性前脫位、肩袖損傷、Hill-Sachs 損傷。術前診斷右肩關節慢性鎖定性前脫位伴肩袖損傷。全關節鏡下徹底松解關節盂、肱骨頭、肩胛下肌腱周圍及肱三頭肌止點軟組織，關節復位后以改良雙袢法彈性固定 Latarjet 術。結果術后切口Ⅰ期愈合，無重要血管及神經損傷等并發癥發生。患者獲隨訪 24 個月。影像學復查示盂肱關節恢復正常對位關系，無脫位復發。右肩關節功能恢復正常，24 個月時 ASES 評分及 Constant-Murley 評分均為 94 分。結論關節鏡下改良彈性固定 Latarjet 術治療慢性鎖定性肩關節前脫位療效滿意。
