目的 探討影響二尖瓣置換術患者體外循環前血容量變化的相關因素及防治措施。方法 選擇四川大學華西醫院2012年1～6月擬行單純二尖瓣置換術患者20例，其中男9例，女11例；年齡（45.6±9.7）歲。排除標準為：（1）紐約心臟病協會（NYHA）心功能分級Ⅲ、Ⅵ級的患者（以避免心功能不全引起的肺淤血和體循環淤血對研究結果的干擾）；（2）合并三尖瓣、主動脈瓣病變和肺動脈疾病的患者（以避免其對中心靜脈壓和血壓的影響）。分別于麻醉誘導前5 min、麻醉誘導后5 min、體外循環前5 min三個時間點監測患者血壓、中心靜脈壓（CVP）、心率改變，分析患者血容量的變化。結果 與麻醉誘導前比較，麻醉誘導后患者收縮壓平均下降20 mm Hg（P<0.05），舒張壓下降6 mm Hg（P<0.05），平均動脈壓下降約10 mm Hg（P<0.05）。至體外循環前，患者血壓，特別是收縮壓進一步下降，與麻醉誘導后比較差異有統計學意義（P<0.05）。麻醉誘導后患者CVP為（3.7±2.0）cm H2O，體外循環前CVP升高至（5.1±2.2）cm H2O，兩者比較差異有統計學意義（P<0.05）。麻醉誘導前心率為（80.5±13.8）次/分，麻醉誘導后增加至（83.5±11.1）次/分，至體外循環前進一步增加至（96.3±12.9）次/分，35%的患者出現竇性心動過速（7/20）。結論 二尖瓣置換術患者體外循環前存在血容量不足，可造成術中嚴重血流動力學紊亂。根據血容量變化進行容量支持治療，可避免體外循環前血流動力學紊亂。
