目的總結肝移植治療肝細胞癌合并膽管癌栓的臨床經驗。方法回顧性分析2003年10月至2012年12月期間于筆者所在醫院接受肝移植治療的3例肝細胞癌合并膽管癌栓患者的臨床資料，總結臨床經驗。結果3例肝細胞癌合并膽管癌栓患者均行改良背馱式原位肝移植術。術后免疫抑制劑方案均采用他克莫司+嗎替麥考酚酯+甲潑尼龍。術后2例行全身化療。2例口服拉米夫定抗病毒治療，1例口服恩替卡韋抗病毒治療。3例患者均聯合應用乙型肝炎免疫球蛋白預防乙肝復發。術后2例分別無瘤生存141個月和38個月，1例于術后3年死于腫瘤復發。結論對于肝細胞癌合并膽管癌栓患者，選擇適當病例施行肝移植手術，可能是一種積極有效的治療措施。
