目的 比較經胸腹聯合切口及經脅腹切口腎切除術治療巨大腎腫瘤的臨床療效。
方法 回顧性分析兩種手術方法治療194例巨大腎腫瘤患者的療效，其中經脅腹切口96例（A組），經胸腹聯合切口98例(B組）。對兩組患者的手術時間、術中失血、腫瘤大小、血管內癌栓清除率、止痛劑使用、術后拔除引流管時間和住院時間等進行統計分析，并對患者長期隨訪和問卷調查（隨訪時間8～30個月，平均22.5個月），比較兩組術后疼痛消失時間、術后停止使用止痛藥時間、術后恢復正常活動時間、術后恢復工作時間等情況。
結果 收回有效問卷A組56例（58.3%），B組60例（61.2%）。兩組手術均獲成功，兩組患者性別構成、年齡具有可比性（P gt;0.05）。A組平均手術時間、術中出血量、腫瘤大小及拔除引流管時間分別為155.6±10.8min、209.9±12.6ml、17.8±2.2cm和3.82±0.66d，B組分別為123.6±4.9min、150.2±10.0ml、20.2±2.2cm、3.27±0.52d，兩組差異有統計學意義（P lt;0.001)。B組血管內癌栓清除率明顯高于A組。而術后使用止痛藥的病例數、下床活動時間、住院時間、術后使用止痛藥時間、術后疼痛消失時間、術后恢復正常活動時間及術后恢復工作等方面兩組差異無統計學意義(P gt;0.05)。
結論 與傳統的經脅腹切口腎切除術相比，經胸腹聯合切口腎切除術具有顯露好、視野開闊，手術時間短、出血少等優點，是一種安全、有效的治療方法。
