目的 觀察體外靜脈靜脈轉流對患者血流動力學的影響。
方法 原位異體肝移植患者14例,術中使用離心泵,部分肝素化方法行體外靜脈轉流。
結果 體外靜脈轉流時間(98.9±23.6)min,轉流量(1 168±260)ml/min,轉流期及新肝5分鐘心排血量(CO)明顯下降,但仍維持在正常水平,體循環阻力(SVR)、肺循環阻力(PVR)轉流30分鐘后明顯增加,心率(HR)、平均動脈壓(MAP)、中心靜脈壓(CVP)、平均肺動脈壓(MPAP)、肺毛細血管楔壓(PCWP)等在轉流期均無明顯變化,新肝早期MAP明顯下降,SVR明顯增加。
結論 無肝期采用離心泵行體外靜脈轉流,有助于穩定血流動力學,但新肝早期仍有明顯血壓下降,主要通過適量快速泵灌注全血來糾正。
