目的 總結法洛四聯癥（TOF）合并冠狀動脈畸形（ACA）手術治療經驗，探討ACA的診斷，右心室流出道（RVOT）疏通手術方式的選擇及其效果。 方法 回顧性分析2004年1月至2010年1月武漢亞洲心臟病醫院手術矯治TOF合并ACA 29例，其中男18例，女11例；年齡7歲（5個月～33歲），體重18 （5～51） kg。術前動脈血氧飽和度65%～91%。所有患者均行完全矯治手術。1例經右心房疏通RVOT，修補室間隔缺損；3例經右心房疏通RVOT，修補室間隔缺損，用自體心包片加寬主肺動脈；3例行肺動脈移位術（REV），5例患者采取“雙通道”手術； 11例采取冠狀動脈下方、上方或者兩邊切口疏通，補片加寬的方法（“單片”或“雙片”法）；6例行冠狀動脈移植術后行跨瓣環補片重建RVOT。 結果 體外循環時間78 （65～102） min，主動脈阻斷時間50 （40～82） min，手術時間150 （126～178） min。手術未出現死亡以及嚴重冠狀動脈損傷。術后2例因出血行二次開胸止血；2例出現低心排血量，強心藥物支持后治愈。隨訪51 （21～83）個月，隨訪過程中無死亡。所有患者術后心功能（NYHA）分級均為Ⅰ級，心臟射血分數均正常，心電圖未提示心肌缺血，動脈血氧飽和度恢復至96%～99%。術后早期RVOT壓差（△P）為19 （8～38） mm Hg，術后隨訪患者無△P增加。 結論 對于TOF合并ACA患者，術前通過64排螺旋CT （64-MSCT）檢查可以明確ACA診斷，術中根據冠狀動脈情況選擇合適的流出道疏通方法，手術效果良好。
