目的:比較低位直腸癌幾種術式的復發率及5年生存率的差異,探討術式的選擇。方法:收集我院2001～2008年收治268例低位直腸癌病例資料,按局部切除,TME+DIXON、TME+MILES分成三組,對術后復發率及5年生存率進行回顧分析。結果:局部切除組12例,2年局部復發2例,復發率16.%,TME+DIXON組192例,總保肛率84.%,2年復發45例,復發率16.%,5年生存率61.%,TME+MILES術64例,2年復發11例,復發率17%,5年生存率59%,三組病例2年復發率相近,統計學數據顯示無顯著性差異(P gt;0.5)。結論:對低位直腸癌,可根據病灶大小,病理學類型,Dukes分期等把握術式選擇的適應癥:保肛術式為首選, Miles術為最后的選擇。
