目的:探討立體定向腦病變活檢術的手術技巧及影響因素。方法:總結20例立體定向活檢病例,其中多發病灶者4例,單發病灶者16例。使用CT、MRI掃描、立體定向儀及手術計劃系統精確定位靶點、制定合適的活檢軌跡。使用Backlund和Sedan活檢針取材,腦淺表病灶使用立體定向環鉆開顱全切。術中冰凍活檢,術后石蠟包埋病理檢查和免疫組化檢查。結果:19例獲明確的病理診斷,活檢陽性率95%。其中,9例腦膠質瘤,3例海綿狀血管瘤、2例腦轉移瘤,3例非特異性炎性肉芽腫,1例結核性肉芽腫,1例腦弓形蟲病。結論:立體定向腦病變活檢術是神經外科十分重要的安全的診治手段。
