達芬奇機器人系統在高齡重度梗阻性黃疸患者中的應用_《中國普外基礎與臨床雜志》

目的　總結達芬奇機器人手術系統在高齡重度梗阻性黃疸患者中的治療經驗。
方法　第二炮兵總醫院肝膽胃腸病研究所在2009年1月至2011年5月期間共行機器人手術209例，其中高齡重度梗阻性黃疸患者47例（機器人組），同期開腹手術患者44例（開腹組），對比觀察患者疾病分布，術前、術中及術后等相關指標，對其結果進行歸納分析。
結果　2組患者基線一致（P＞0.05）。機器人組46例在機器人輔助下完成手術，手術完成率達97.87%（46/47），1例中轉開腹，中轉率為2.13%（1/47）；手術方式包含肝膽胰外科所有手術方式，且其中1例肝門部膽管癌患者因血清膽紅素水平極高（375μmol/L），急診行肝門部膽管癌切除＋肝門-空腸Roux-en-Y吻合術；術后有7例患者出現并發癥，總體并發癥發生率為14.89%（7/47）；圍手術期死亡1例，死亡率為2.13%（1/47）；術后3個月生存率為78.15%；術后15 d和術后3個月體力狀況評分分別為42.87±18.61和58.51±23.86；腹部切口總長度為（6.30±1.70） cm。開腹組所有患者均完成手術，其中急診手術4例；術后13例患者出現并發癥，總體并發癥發生率為29.55%（13/44），圍手術期死亡2例，死亡率為4.55%（2/44）；術后3個月生存率為72.36%；術后15d和術后3個月體力狀況評分分別為37.15±13.64和45.27±18.96；腹部切口總長度為（26.73±3.07） cm。2組總體并發癥發生率、術后15d和術后3個月體力狀況評分及切口總長度比較，差異均有統計學意義（P＜0.05）。
結論　達芬奇機器人手術系統對于高齡重度梗阻性黃疸患者的治療是安全、有效的，其創傷小，操作靈活精準，術后恢復快，拓展了外科手術的年齡界限，擴大了手術適應人群。
