目的 探討鎮靜、鎮痛麻醉在治療性ERCP中的安全性及臨床應用。
方法 選取在蘭州大學第一醫院普外二科ERCP診療中心行治療性ERCP患者1 660例，分為靜脈鎮靜、鎮痛組（n=800）和常規鎮靜、鎮痛組（n=860），觀察各組患者麻醉前、麻醉后、術中、麻醉清醒后的心率、血壓、呼吸和血氧飽和度的變化，并應用Ramsaymin分級法評估麻醉狀態，記錄患者術中嗆咳、躁動、患者對手術過程有無不良記憶、術后腹痛等相關不良反應的發生。
結果 與常規鎮靜、鎮痛組相比，靜脈鎮靜、鎮痛組患者圍手術期各項生命體征平穩。對于術后不良反應的發生率，靜脈鎮靜、鎮痛組患者出現腹痛占4.4%(35/800)、腹脹占2.6%(21/800)，惡心和嘔吐占3.6%(29/800)； 而常規鎮靜、鎮痛組分別為36.3%（312/860）、49.0%（421/860）和53.0%(456/860)，2組差異有統計學意義（P＜0.01）。靜脈鎮靜、鎮痛組患者術后滿意度為96.9%(775/800)，不良反應回憶度為4.8%(38/800)； 而常規鎮靜、鎮痛組患者術后滿意度僅為2.9%(25/860)，不良反應回憶度卻為97.9%(842/860)，2組差異有統計學意義（P＜0.01）。
結論 舒芬太尼+咪達唑侖+連續輸注丙泊酚在治療性ERCP中具有良好的鎮靜、鎮痛麻醉效果，值得臨床推廣。
