        • Magnetic Resonance Imaging in Female Pelvic Masses:A Meta-Analysis

          Objective To evaluate the sensitivity, specificity, and accuracy of magnetic resonance imaging (MRI) in characterizing adnexal masses. Methods The databases such as the Cochrane Library, PubMed, EMbase, CNKI, and WanFang Data were searched on computer from 1991 to 2011. The reviewers screened the trials according to inclusion and exclusion criteria strictly, extracted the data, and assessed the methodology quality. Meta-analysis were performed using the Metadisc 1.40 software. The acquired pooled sensitivity, specificity, and summary receiver operating characteristic curve (SROC) were used to describe the diagnostic value. The pooled likelihood ratios were calculated based on the pooled sensitivity and specificity. Results Ten case-control studies involving 649 women who were suspected to have pelvic masses were included and 729 masses were confirmed by the postoperative histopathology. The pooled statistical results of meta-analysis showed that:the sensitivity and specificity of MRI were 〔89%(84%-92%), P=0.046 6〕 and 〔87% (83%-90%), P=0.000 2〕 respectively, the positive and negative likelihood ratios of MRI were 6.25(P=0.008 5) and 0.14(P=0.029 1) respectively, and the area under the SROC curve (AUC) was 0.941. The sensitivity and specificity of ultrasound were 〔87%(82%-91%), P=0.000 0〕 and 〔73%(69%-77%), P=0.000 0〕 respectively, the positive and negative likelihood ratios of MRI were 3.07(P=0.000 0) and 0.18(P=0.000 1) respectively, and the AUC was 0.897. The speci?city and accuracy of MRI in characterizing female pelvic masses were higher than ultrasound obviously. Conclusion According these evidences, the MRI should be recommended to the women who are suspected to have pelvic masses as a preferred.

        • Sex hormone levels have little impact on outcomes of cardiac surgery in fertile women with congenital heart disease: A retrospective cohort study

          Objective To examine the influence of hormonal fluctuations on the perioperative outcomes of patients undergoing congenital heart surgery. Methods We conducted a retrospective analysis of clinical data from fertile women diagnosed with congenital heart disease at the Guangdong Provincial People's Hospital, between January 1, 2015, and July 30, 2019. Initially, patients were categorized into groups based on serum progesterone levels: a low progesterone group (n=31) and a high progesterone group (n=153). Furthermore, based on serum estrogen levels, they were divided into a low estrogen group (n=10), a medium estrogen group (n=32), and a high estrogen group (n=118) for comparative analysis. A control group (n=24) consisted of patients who received progesterone injections before their menstrual period. Results We finally included 184 patients. The patients’ average age was 27.6±5.7 years, with 142 (77.17%) presenting with complex congenital heart conditions. There were statistically significant differences in total postoperative standard thoracic drainage volume and postoperative albumin level between the high and low progesterone groups (P<0.05), while other perioperative outcome indicators showed no statistical differences (P>0.05). Among the different serum estrogen level groups, there were statistically significant differences in postoperative blood urea nitrogen levels, total postoperative standard thoracic drainage volume, and hospital stay (P<0.05), while other perioperative outcome indicators showed no statistical differences (P>0.05). ConclusionConsidering the overall clinical significance, the physiological changes in sex hormone levels appear to have a negligible effect on the perioperative outcomes of fertile women with congenital heart disease.

        • Risk Factor Analysis for Osteoporosis in Elderly Women

          ObjectiveTo observe the risk factors for osteoporosis in elderly women in Chengdu Shuangliu County, in order to provide a reference for the prevention of this disease. MethodsThrough random cluster sampling from 6 communities in Shuangliu County, 276 aged (60-84 years old) women accepted questionnaire survey from May 2010 to October 2012. Their bone mineral density was also measured. ResultsThe osteoporosis rate in these 276 elderly women was 49.64%. Different occupations had a significant impact on the occurrence of osteoporosis (P<0.05). Osteoporosis rate in elderly women in rural areas was higher than that in urban areas, but there was no significant difference (P>0.05). The rate of women who paid attention to calcium and vitamin D preparations was significantly lower than those with no supplement of calcium and vitamin D preparations (P<0.05). Single factor analysis showed that age, body mass index of age 30, present weight, menopause age, suffering from respiratory diseases were closely related to osteoporosis (P<0.05). Multivariate analysis showed that age, present weight, consumption of milk, menopause age, and respiratory disease history were closely correlated with osteoporosis incidence (P<0.05). ConclusionThe incidence of osteoporosis in women at age 60 or older in Chengdu Shuangliu County is high. Medical workers should strengthen the health education for those elderly women with osteoporosis and promote a healthy lifestyle and a balanced diet.

        • The prevalence of high risk human papillomavirus of females in western China: a meta-analysis

          ObjectivesTo systematically review the epidemiology of high-risk human papillomavirus (HPV) infections in western Chinese females.MethodsPubMed, Web of Science, The Cochrane Library, CNKI and WanFang Data databases were electronically searched to collect epidemiological studies on female genital high-risk HPV infection in western China from January 2000 to July 2018. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies, and then, meta-analysis was performed by using R software.ResultsA total of 35 studies involving 149 037 objects were included. The results of meta-analysis showed that: the total infection rate of high-risk HPV in the northwest was 12.21% (95%CI 10.0% to 16.72%), and that in the southwest was 17.48% (95%CI 13.55% to 21.4%). The infection rate of high-risk HPV among healthy females in the northwest was 10.03% (95%CI 10.0% to 11.67%), while that in the southwest was 14.94% (95%CI 11.51% to 18.38%). CINⅠ, CINⅡ-Ⅲ and cervical cancer patients in the northwest with high-risk HPV infection rate were 70.31% (95%CI 49.0% to 91.61%), 84.29% (95%CI 68.36% to 100.22%) and 89.35% (95%CI 74.15% to 104.55%), respectively. The infection rates of patients with corresponding lesions in the southwest were 59.06% (95%CI 45.87% to 72.25%), 83.79% (95%CI 76.62% to 90.96%), and 81.07% (95%CI 67.77% to 94.37%), respectively.ConclusionsCurrent evidence shows that the epidemiological pattern of female genital high-risk HPV infection in western China is basically consistent with overseas. The high-risk HPV subtypes are subtype 16, 18, 31, 52, 53 and 58. The HPV vaccine used in China has basically covered the prevalent high-risk HPV subtypes in Western China. The wide application of vaccine may reduce the incidence and mortality of cervical precancerous lesions and cervical cancer, which will ensure reproductive health females in Western China. Due to limited quality and quantity of the included studies, more high quality studies are required to verify above conclusions.

        • The association between radiotherapy and the risk of second primary lung cancer among female breast cancer patients: A systematic review and meta-analysis

          ObjectiveTo explore the association between radiotherapy and the risk of subsequent primary lung cancer in female patients with breast cancer. MethodsThe EMbase, Web of Science, PubMed, CNKI, VIP and WanFang databases were searched from the establishment date to July 1, 2021. The relative risk (RR) with 95% confidence interval (CI) were combined and all statistical analyses were performed by STATA 12.0 software. ResultsA total of 10 publications including 466 510 participates from 14 studies were included. The meta-analysis indicated that radiotherapy was a risk factor for the occurrence of second primary lung cancer among female breast cancer patients [RR=1.45, 95%CI (1.18, 1.78), P<0.001]. Subgroup analysis based on the relative position of breast cancer and lung cancer was conducted and the results demonstrated that radiotherapy only increased the incidence rate of ipsilateral lung cancer [RR=1.27, 95%CI (1.10, 1.45), P=0.001], without significant change of the risk of contralateral lung cancer [RR=1.16, 95%CI (0.77, 1.74), P=0.487]. ConclusionRadiotherapy is one of the risk factors for subsequent primary pulmonary carcinoma among female breast cancer patients, especially for ipsilateral lung carcinoma. However, more high-quality studies are still needed to verify above findings.

        • CLITOROPLASTY FOR GENITAL VIRILIZATION IN GIRLS WITH CONGENITAL ADRENAL HYPERPLASIA

          ObjectiveTo investigate the effectiveness of clitoroplasty of reserving the clitoris dorsal neurovascular bundle and the ventral complete urethral plate for genital virilization in girls with congenital adrenal hyperplasia (CAH). MethodsBetween June 2000 and October 2015, 27 girls with genital virilization were treated with clitoroplasty of reserving the clitoris dorsal neurovascular bundle and the ventral complete urethral plate. The chromosomal karyotype was 46, XX in all patients, whose age ranged from 3 to 11 years (mean, 3.9 years). According to Parder's virilization scale, 5 cases were classified as stage Ⅱ, 18 cases as stage Ⅲ, and 4 cases as stage Ⅳ. The hormone tests showed decreased cortisol level and increased testosterone and 17-hydroxyprogesterone levels. They were diagnosed with non-salt-wasting CAH. ResultsAll of the patients underwent clitoroplasty successfully. The mean operation time was 74 minutes (range, 58-95 minutes). Incision healed primarily. The patients were followed up 6 months to 10 years (median, 19 months). The external genitalia had good appearance, without necrosis of flap and the glans clitoris, abnormal sensation of the glans clitoris, or clitorism recurrence. Five girls had breast development and normal menstrual cycle during adolescence, and 1 patient who was followed up for 10 years gave birth to a son. ConclusionClitoroplasty of reserving the clitoris dorsal neurovascular bundle and the ventral complete urethral plate is a relatively ideal method for treating genital virilization in girls. It has less complications, good aesthetic and functional results. There are satisfactory outcomes when combining with the endocrine treatment.

        • The Female Estradiol Level Change in Recent Years

          目的:檢測血清雌二醇(E2)的水平,在一定程度上了解女性生育能力及身體健康狀況。探討各種致病因子對人體的損害。方法:放射免疫全定量分析,統計學上采用百分比統計。結果:筆者對近三年1164例有生殖系統臨床癥狀(主要表現為月經不調、量少或停經等),年齡在18歲至40歲間的育齡女性患者的雌二醇-E2檢測結果進行了回顧性總結分析,發現其激素水平呈逐年下降趨勢,年齡也趨向相對低齡趨勢。結論:上述結果可能在一定程度上反映了生活環境、工作環境、生活習慣、食物、藥物及其他疾病等因數都會對人體內分泌及生殖系統的生理健康造成一定程度的影響甚至產生嚴重疾患。
