Objective To evaluate the diagnostic value of magnetic resonance cholangiopancreatography (MRCP) in obstructive jaundice. Methods Forty eight consecutive patients with obstructive jaundice were examined by MRCP, all results were testified during and /or after operation. Results Different causes of obstruction had their own characteristic manifestations. Level of obstruction was accurate in 100%, the accuracy in distinguishing obstructive causes was 93.8%. Conclusion MRCP is quite effective, safe and reliable in diagnosis of obstructive jaundice.
Objective
To clarify relationship between signal heterogeneity on hepatobiliary phase of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced MRI and prognosis of hepatocellular carcinoma (HCC).
Methods
From January 2014 to January 2017 in the First Affiliated Hospital of Chongqing Medical University, a total of 77 patients with the pathologically proved HCC underwent Gd-EOB-DTPA-enhanced MRI prior to surgery were included in this study. On the basis of the signal heterogeneity in the hepatobiliary phase, the included patients were designed to homogeneous hypointensity group and heterogeneous hyperintensity group. The disease-free survival time were compared between the 2 groups and it’s influencing factors were analyzed.
Results
Seventy-seven patients with HCC were included, including 45 cases of homogeneous hypointensity and 32 cases of heterogeneous hyperintensity. There were no significant differences in the age, gender, etiology, liver function, alpha-fetoprotein, differentiated degree, Child-Pugh grade, lesion diameter, lesion border, and number of lesions between the 2 groups (P>0.05). However, the HCC patients with heterogeneous hyperintensity had a later BCLC staging (P=0.001). The disease-free survival time of the patients with homogeneous hypointensity and heterogeneous hyperintensity was (17.0±9.8) months and (12.4±10.4)months, respectively. The Kaplan-Meier survival curve showed that the disease-free survival time in the patients with homogeneous hypointensity was significantly better than that in the patients with heterogeneous hyperintensity (P=0.020). The results of univariate analysis showed that the other confounding factors had no effect on the disease-free survival time of patients with hepatocellular carcinoma (P>0.05) except for the signal of hepatobiliary phase (P<0.05). Furthermore, the hepatobiliary phase signal, BCLC stage, and degree of differentiation, which might be clinically considered as potentially influencing for the prognosis of patients with HCC, were included in the Cox multivariate proportional hazard regression model and found that the heterogeneous hyperintensity was still the risk factor of the disease-free survival rate in patients with HCC (P=0.047).
Conclusion
Signal heterogeneity on hepatobiliary phase of Gd-EOB-DTPA-enhanced MRI is related to prognosis of patients with HCC, heterogeneous hyperintensity may indicate a lower disease-free survival rate.
ObjectiveTo investigate the value of MRI in the diagnosis of central nervous system lymphoma (CNSL).
MethodsWe retrospectively analyzed the clinical data of 20 cases of primary CNSL (PCNSL) and 13 cases of secondary CNSL (SCNSL) from the Second People's Hospital of Chengdu and Chengdu 363 Hospital from January to December 2013, and analyzed their clinical data and MRI image data. We observed the tumor location, tumor size and signal, and carried out the statistical analysis.
ResultsTwenty patients had PCNSL in the brain, including single lesion in 9 (45.0%), and multiple in 11 (55.0%). Among the 48 lesions, there were 23 (47.9%) nodular lesions, 21 (43.8%) crumb lesions, and 4 (8.3%) dot patch lesions; MRI showed slightly low T1 signal and slightly high T2 signal in most lesions, and showed significant even enhancing, and mild to moderate edema around the tumor. SCNSL lesions were mainly meningeal disseminated with 3 cases (23.1%) of single lesions and 10 cases (76.9%) of multiple ones, and there were a total of 30 lesions. MRI manifested that T1 and T2 mainly showed equal signals, and showed an obviously even enhancing status, and mild to moderate edema around the tumor.
ConclusionThe central nervous system lymphoma has a certain characteristic MRI image, and MRI images of the primary and secondary central nervous system lymphoma were similar.
Focus on the inconsistency of the shape, location and size of brain glioma, a dual-channel 3-dimensional (3D) densely connected network is proposed to automatically segment brain glioma tumor on magnetic resonance images. Our method is based on a 3D convolutional neural network frame, and two convolution kernel sizes are adopted in each channel to extract multi-scale features in different scales of receptive fields. Then we construct two densely connected blocks in each pathway for feature learning and transmission. Finally, the concatenation of two pathway features was sent to classification layer to classify central region voxels to segment brain tumor automatically. We train and test our model on open brain tumor segmentation challenge dataset, and we also compared our results with other models. Experimental results show that our algorithm can segment different tumor lesions more accurately. It has important application value in the clinical diagnosis and treatment of brain tumor diseases.
Objective To observe the results of function MRI and perimetry in patients with visual pathway diseases.Methods Three patients (6 eyes) with pituitary adenoma and craniopharyngioma diagnosed via pathological examination and three healthy volunteers aged from 24 to 30 were collected. The best corrected visual acuity was nonlight perception1.0 in the 6 sick eyes and 1.0 in the healthy eyes; all the involved individuals had no other ocular diseases except myopia and without any contraindications of MRI. Common tests including the best visual acuity, fundus test by direct or indirect ophthalmoscope, center static visual field tested by Octopus 101 perimeter, program 32, tendency oriented perimetry were performed. The visual stimulation subtended a field of view of about 12 degrees,consisted of high contrast and drifting checkerboards. MRI parameters: GE signa VH/i 30T scanner. Functional data: GRE-EPI sequence,20 slices lying perpendicular to the calcarine sulcus. Anatomical data was obtained using 3DSPGR sequence to acquire high resolution. The cortical surface was unfolded and then cut and inflated. Functional data was presented to the inflated surface and subsequently analyzed by AFNI software.Results In six eyes, three had temporal defects, two had upper temporal visual field defects, and the other one did not finish the visual field test. The retinotopic representations of health adults were obtained by using the phaseencoded visual stimulation. The Eccentricity coordinate maps showed that foveal representations lay in the occipital poles and the representations appeared further anterior as eccentricity increased. The polar angle coordinate maps showed that early retinotopically organized areas had a representation of visual field. The visual cortex beneath the calcarine sulcus matched with the upper visual field of the opposite side and which upon the calcarine sulcus matched with the under visual field of the opposite side. Less or no visual cortex response was revealed in the patients′ function MRI or the response in injury side was vanished. The visual cortex response related with the visual field defects could not be induced in function MRI.Conclusion There is a good correlation between function MRI data and the results of perimetric evaluation. The function MRI can show the visual cortex response correlated with the visual field defects of the patients with visual pathway diseases.
Objective To observe the changes of visual pathway of central nervous system in patients with glaucoma.Methods Thirty-five subjects were enrolled in this prospective study. There were seven patients of chronic primary angle closure glaucoma with bilateral visual field defects, 12 patients of primary open angle glaucoma with bilateral visual field defects, and 16 normal subjects. GE Signa HD 1.5 T magnetic resonance scanners and head eight phased array were used. The mean fractional anisotropy (FA) and the mean average diffusion coefficient (DCavg) of white matter tracts in visual pathway of bilateral optic nerve, optic chiasma, bilateral optic tract,bilateral optic radiation were measured by diffusion tensor imaging, meanwhile the white matter tracts were reconstructed by fiber tracking system. The volumes of lateral geniculate body were measured by coronal proton density weighted magnetic resonance imaging.Results The differences of FA among bilateral optic nerve, optic chiasma, bilateral optic tract,bilateral optic radiation were statistically significant (F=25.985,20.626,12.262,22.399,21.708,24.994,22.774;P<0.05). There was no difference of DCavg among bilateral optic nerve, optic chiasma, bilateral optic tract,bilateral optic radiation(F=2.097,2.178,0.530,0.983,0.608, 0.866, 1.504;P>0.05). The differences of volume of lateral geniculate body among three groups were statistically significant (F=18.631,17.274;P<0.05). Conclusion There is degeneration in visual pathway of central nervous system in patients with glaucoma.
目的 探討囊性腦膜瘤的磁共振表現、表觀彌散系數(ADC)圖的影像特點、ADC值與病理分級的關系及ADC值對判斷不同病理亞型腦膜瘤的價值。 方法 回顧性分析2003年3月-2007年12月18例經病理證實的囊性腦膜瘤患者的磁共振表現,其中男8例,女10例,平均年齡45.6歲。在ADC圖上分別測量腫瘤實質、瘤周水腫、囊變區的平均ADC值(均取4~6個區域,取平均值),比較不同病理亞型、不同病理分級的腫瘤實質、瘤周水腫、囊變區的ADC值差異是否有統計學意義。 結果 良惡性腦膜瘤腫瘤實質ADC值差異無統計學意義(P>0.05);囊性腦膜瘤各亞型間的腫瘤實質、瘤周水腫的ADC值差異無統計學意義(P>0.05);腫瘤實質、囊變區與瘤周水腫平均ADC值相比差異均有統計學意義(P<0.05)。 結論 ADC值可區分腫瘤實質、囊變區及瘤周水腫,但對腦膜瘤亞型及良惡性的甄別需結合常規MRI與增強的征象。Objective To explore the MRI imaging manifestation, features of apparent diffusion coefficient (ADC) map and the relationships among ADC value, pathologic grading and pathologic subtype of cystic meningioma. Methods The clinical data of 18 patients (8 males and 10 females, with an average age of 45.6 years) with cystic meningiomas confirmed by pathologically examination were retrospectively analyzed. The ADC values of tumor parenchyma, peritumoral edema, cystic regions and the contralateral normal brain of the tumor parenchyma were measured and analyzed.The ADC value of the tumor parenchyma, peritumoral edema, cystic region were measured and then compared with the pathological findings. P value of less than 0.05 was considered as having a statistically significant difference. Results There was no significant difference in ADC values between benign and malignant meningioma and also among the tumor parenchyma, periedema and cystic region in different pathological subtypes. The difference in ADC values amongst tumor parenchyma, peritumoral edema and cystic region were statistically significant (P<0.05). Conclusions Although ADC quantitative measurement allows the differentiation of the tumor parenchyma, cystic region and peri-edema, it is need to combined with conventional MR examination to assess tumor malignancy as well as grading in cystic meningioma.
【Abstract】ObjectiveTo investigate the relationship of magnetic resonance diffusion-weighted imaging (DWI) to histology in the patients of chronic viral hepatitis. MethodsThirty-five patients of chronic viral hepatitis who received liver biopsy and 10 healthy volunteers were included in this study. All of them underwent DWI on a 3.0T MRI device. Apparent diffusion coefficient (ADC) of the liver were measured respectively when b value were set as 100, 400, 600 and 800 s/mm2. Biopsy specimens were scored for fibrosis and necroinflammation according to the Knodell histology activity index (HAI). ResultsWhen b value was set as 800 s/mm2, statistical difference was showed between the fibrosis group and the nonfibrosis group, statistical difference was also shown among the different degrees of necroinflammation and fibrosis. ConclusionDWI is a valuable method for grading and staging of chronic viral hepatitis.
Objective To detecte the pathogenetic mechanism of cortical deficit in persons with strabismic amblyopia by blood oxygenation level dependent-functional magnetic resonance imaging(BOLD-fMRI). Methods The data of BOLD-fMRI of occipital visual cortex in 11 persons with strabismic amblyopia and 8 healthy ones were collected by 1.5T MRI system.The results of binocular pixel exponent in strabismic amblyopia group was compared with that in the normal group and the result of and average activation of cortical neuron in strabismic amblyopia group was analyzed.Results The binocular pixel exponent was lower in the strabismic amblyopia group(14.13%±4.55%)than that in the normal group(47.82%±5.34%)(PConclusions The reduction of cortical binocular cells and the undersampling and coding of higher-spatial-frequency components of visual stimuli may be related to the strabismic amblyopia.BOLD-fMRI may provide a new path to detecte the pathoffenetic mechanism of cortical deficit in people with strabismic amblyopia.
