        • DEVISING AND CLINICAL APPLICATION OF LADDERSHAPED OSTEOTOMY AND GUIDE APPARATUS

Objective To explore devising and clinical effect of ladder-shaped osteotomy and guide apparatus. Methods From February 2002 to January 2004, with the application of guide apparatus devised, the ladder-shaped osteotomy was designed and completed by bone drill and linear saw. The experiment of ladder-shaped osteotomy was carried out on a fresh bone trunk amputated. Clinically, 3 cases were replanted by shortening humerus by means of laddershapedosteotomy. The lengths of bone shortened were 3 to 7 cm. The bone length of ladder-shaped osteotomy was 2 to 3 cm. Two cases of radius defect was repaired with free fibula. The lengths of bone transplanted were 7 and 11 cm. The bone length of ladder-shaped osteotomy was 1 cm. Results It took 2 to 3 minutes to complete the laddershaped osteotomy on the two ends of bone defect. The ends of ladder-shaped osteotomy was integrated closely. All 5 cases were followedup for 10 to 16 months. All fractures healed during 14 to 20 weeks. Of 3 cases shortening humerus, the function of elbow joint was normal in 1 case, the activity range of elbow joint was 0 to 100° in 1 case, and 0 to 80° in 1 case, respectively. Of 2 cases undergoing fibula transplantation, the function of wrist and elbow joint were normal;the pronation and supination ranges of the forearm was 30° and 40° in one, 50° and 45° in the other. Conclusion With the introduction of guide apparatus, the laddershaped osteotomy by bone drill and linear saw is a recommendable procedure because of many advantages such as simple apparatus, shortcut, laborsaving and precision.

        • RESEARCH PROGRESS OF PLATELET-RICH PLASMA IN PROMOTING BONE REGENERATION AND REPAIRING

Objective To investigate the factors that affect platelet-rich plasma (PRP) in promoting bone regeneration and repairing. Methods Recent l iterature was reviewed, concerning the preparations of PRP, physiological mechanism and the latest appl ications in orthopedic field. Results PRP, the concentrated body of autologous platelet, was rich in platelets and was the source of autologous growth factors. Many studies had shown that PRP played an important role in promoting bone regeneration and repairing. However, a few experimental results contradicted this point. The reason might be that the biological properties of PRP were influenced by various factors, such as workmanship, vector, activation schemes, working concentration, individual difference. Conclusion The concentration and qual ity of platelet and other related factorsof PRP affect the rel iabil ity of the results and conclusions. So an efficient and stable production method of PRP should beestabl ished.

        • Analysis on Effect of Using Hemoglobin-revised Trauma Score on Predicting the Prognosis of Emergency Trunk Injury within 28 Days

ObjectiveTo investigate the effect of using hemoglobin-revised trauma score (HB-RTS) on predicting mortality risk in trunk injury patients. MethodsAccording to case control study rules, medical records of patients with trunk injury who were treated between July 2011 and October 2013, were collected. Patients were divided into two groups according to their survival prognosis within 28 days. The HB-RTS, revised trauma score (RTS), and injury severity score (ISS) of the two groups were calculated. The receiver operating characteristic (ROC) curve was drawn, and the area under the curve (AUC) and Youden's index were calculated. And then, we selected the optimal cutoff value using HB-RTS and RTS to predict the trunk injury patients' mortality and calculated their sensitivity, specificity, and accuracy. In the end, the correlation of ISS with HB-RTS and RTS using linear correlation analysis method was analyzed. ResultsBesides RTS, HB-RTS and ISS in the death group were statistically significant higher than that in the survival group. The AUC of HB-RTS and RTS of the two groups were 0.922 and 0.888, respectively. The sensitivity of HB-RTS and RTS were 91.5% and 83%; and the specificity of HB-RTS and RTS were 95.7% and 76.6%, respectively. The correlation of HB-RTS and ISS was -0.592, while the correlation of RTS and ISS was 0.585. ConclusionCompared to RTS, ROC of HB-RTS is enlarged, the specificity is increased, and the sensitivity is decreased. HB-RTS has stronger correlation with ISS than with RTS. The predictive effect of HB-RTS is higher than RTS in predicting mortality of trunk injury patients.
