        • GROIN SKIN AND ILIAC BONE COMPOSITE GRAFT WITH VASCULAR PEDICLE IN THE REPAIR OF SEVERE DEEP BURN AND BONE DEFECT OF HAND AND WRIST

          Abstract From March, 1987 through May, 1996, a total of 13 cases of severe deep burn and bone defect of hand and wrist were treated by groin skin and iliac bone composite graft with vascular pedicle and had resulted in satisfactory result. The operation was relatively simple. Because thecomposite graft carried its own blood supply in the pedicle, it was not necessary to revascularize the composite graft by anastomosis of blood vessel during operation. Owing to the presence of abundant vascular supply of the iliac bone, the antiinfection potency was high, so its application was suitable for those conditions such as fresh severe deep burn with infection and bone defect. As a result, this technique gave the best chance to save the limb from amputation, and the duration required for treatment could be markedly shortened. This method provided the possibility to solve effectively the difficult problem dealing with the treatment of severe deep burn with infection and bone defect of the hand and wrist.

        • MANUFACTURE AND APPLICATION OF A NEW COMPOSITE ALLOGRAFT

          It is in urgent need clinically to look for an ideal substance for the coverage of burn wounds owing to shortage of autografts or allografts. After the cadaveric skin was extracted with acetic acid, salted out with NaCl and freeze-dried to prepare a porous collagen membrane. The membrane was seeded with allo-epidermal cells and allo-fibroblasts on its two sides, respectively, and then was cultured to achieve an artificial composite allograft. The artificial composite allograft was then transplanted onto ten severly burned wounds. One-year follow-up showed satisfactory results and the histological examination confirmed that the composite allograft could improve the adherence and growth of the epidermal cells and was helpful for blood vessels and healing of non-inflammatory connective tissues in the wounds.

        • ACCELERATION OF BURN WOUND HEALING WITH TOPICAL APPLICATION OF RECOMBINANT HUMAN EPIDERMAL GROWTH FACTOR OINTMENTS

          OBJECTIVE: To investigate the efficiency of recombinant human epidermal growth factor (rhEGF) on burn wound healing and to explore the effective density of the ointments. METHODS: A total of 120 cases of burn in superficial II degree and profound II degree were randomly divided into 2 groups. In the first group of 15 cases of superficial II degree, the wounds were treated by rhEGF ointments of different density, 0.5 microgram/g, 10 micrograms/g and 50 micrograms/g, to screen out the effective density. And in the other 105 cases of the second group, optimal density of the ointments based on the result of the first group were employed to treat the burn wound in superficial II degree and profound II degree, with the self-corresponding wounds of the same degree as control, to study the efficiency of rhEGF on wound healing, according to the wound healing time, and adverse reaction of the ointment. RESULTS: In the first group, the average healing time of superficial II wound treated by ointments of 10 micrograms/g and 50 micrograms/g significantly shortened when compared with that treated by ointments of 0.5 microgram/g(P lt; 0.01), but there was no obvious difference between the cases treated by ointments of 10 micrograms/g and 50 micrograms/g. In the second group, the healing time of superficial II wound treated by ointments of 10 micrograms/g was (8.39 +/- 2.25) days, (9.52 +/- 2.56) days in the control (P lt; 0.01); and healing time of profound II burn treated by ointments of 10 micrograms/g was (16.80 +/- 2.99) days, (18.27 +/- 3.17) days in the control (P lt; 0.01). And healing rates of burn wound at different periods were higher than those of the control. CONCLUSION: The above results indicate that rhEGF ointments can enhance burn wound healing significantly, and the ointment of 10 micrograms/g is a good choice for clinical application.

        • EXPERIMENTAL STUDY OF THE EFFECT OF DANSHEN ON THE VIABILITY OF BURNED SKIN

          In order to investigate the influence of Danshen on the viability of skin from II degree burned and the effect of Danshen on the wound healing, 20 rabbits having a wound from second degree burn were divided into two groups randomly. The experimental group was treated with Danshen and the control was treated with normal saline. Then succinate dehydrogenase, SDH activity, malonyl dialdehyde, MDA concentration, water content and morphological change of wound tissue were observed dynamically. The results showed that the levels of SDH activity in the experimental group were significantly higher than that of the control group (P lt; 0.05), the MDA concentration and water content of the former were lower than that of the latter (P lt; 0.05) and the morphological examination showed that the wound healing process of former were better than that of the latter. The conclusion was Danshen had a beneficial effect on the viability of the burned skin and this effect was due to its protecting the residual epithelial cells of the burned skin.

        • Growth Hormone for Burn Patients: a Systematic Review of Randomized Controlled Trials

          Objective To assess the effectiveness and the safety of clinical use of growth hormone (GH) in burn patients. Method Search were applied to the following electronic databases: Chinese Bio-medicine Database (CBM), MEDLINE, EMBASE and Cochrane Library. Language was restricted in Chinese and English. Data were extracted and evaluated by the two reviewers independently of each other. Applied RevMan 4.1 for statistical analyse. Results Nine trials involving 732 patients were included. The combined results showed that GH can shorten wound healing time [weighted mean difference (WMD) = -11.25, 95%CI (-14.84 to -7.66), Plt;0.000 01], donor site healing times [WMD= -1.87, 95%CI (-2.28 to -1.47), P<0.000 01), and length of hospital stay [WMD= -8.10, 95%CI (-10.40 to -5.79), P<0.000 01]. There was no statistical significance on resting energy expenditure [WMD= -0.04, 95%CI ( -0.08 to 0.00), P=0.06], mortality [odds ratio (OR) =1.15, 95%CI (0.15 to 8.53), P=0.9], sepsis [OR=1.08, 95%CI (0.50 to 2.34), P=0.8] and ventilatory support required [OR=1.51, 95%CI (0.72 to 3.16), P=0.3]. Nevertheless, the plasma levels of glucose [standardized mean difference (SMD) =0.98, 95%CI (0.54 to1.42), P<0.000 01] and insulin [SMD=0.86, 95%CI (0.43 to1.30), P=0.000 1] were increased in GH groups. Conclusions GH for burn patients is effective and safe if blood glucose can be controlled well.

        • REPAIR OF FINGER DEEP BURN WITH DIFFERENT KINDS OF SKIN FLAPS

          To investigate an effective method in clinical application of using different kinds of skin flaps for repair of the finger deep burns. Methods The groin skin flap, the paraumbilical skin flap, the volar digital advancement flap, the island flap from the dorsum of the index finger, the lateral digital neurovascular island flap, and the island skin flap nourished by the cutaneous nerve nutrient vessel of the dorsum were employed to repair 157 fingers in 101 patients (78 males, 23 females, aged 12-56 years, averaged 34.6 years) from January 1997 to December 2006. Of the 101patients, 37 had a deep partial thickness burn involving 59 fingers, and 64 hada full thickness burn involving 98 fingers. The soft tissue defects ranged in area from 1.0 cm×1.0 cm to 6.5 cm×6.0 cm. The interval between the injury and the operation was 4 hours to 5 days in 89 patients, and 18 to 27 days in the other 12 patients who also had infected wounds. The flaps ranged in size from 1.2 cm×1.2 cm to 7.8 cm×6.5 cm. The donor site was directly sutured in 84 patients, and the donor site was covered by a full thickness skin graft in the other 17 patients. Results After operation, 98 patients had an incision healing by first intention and the flaps survived well; the other 3 patients had congestion and necrosis in the flap edges, and had a delayed healing after the dressing changes. All the donor sites had a healing by first intention. The followup of all the patients for 224 months averaged 6.5 months revealed that 9 patients, who had been given the paraumbilical skin flap, had a fat and clumsy finger; 14 patients, who had been given the groin skin flap, also had a fat and clumsy finger; 3 patients developed congestion and necrosis at their edges. The remaining patients had a satisfactory survival of the skin flaps and a normallyshaped finger. The flaps had a good appearance, with the twopoint discrimination of 510 mm, the good finger motion ability, and the satisfactory finger appearance.Conclusion The volar digital advancement flap,the island flap from the dorsum of the index finger, the lateral digital neurov ascular island flap, and the island skin flap nourished by the cutaneous nerve nutrient vessel of the dorsum are good skin flaps for repair of the finger deep burns. The groin skin flap and the paraumbilical skin flap are also good skin flaps for repair of the deep burns of the mutiple fingers but the postoperative finger may become a bit fat and clumsy.

        • EFFECT OF AUTOSKIN GRAFTING IN FAULT HYPODERMIS WOUND OF GRANULATION EXCISION ON FULL-THICKNESS BURN HEALING

          Objective To explore the mechanism of full-thickness burn wound healing with autoskin grafting in fault hypodermis wound of granulation excision and to evaluate its effect.Methods By the techniques of clinical observation, histopathology, immunohistochemistry,TEM and FCM,we observed changes of the activity andstructure of grafted skin and the granulation tissue,collagnous fiber,microvessels,the ultramicrostructure of fibroblasts and the expression of basic fibroblast growth factor(bFGF) in the base of autoskin grafting in fault hypodermis wound in burned adult minipigs(Group A), and compared with traditional method of autoskingrafting on the basilar fibrous tissue wound of scraped partly granulation being(Group B) and control group (Group C, without treatment except de-fur).Results The grafted skin survived after 3 days of operation, and it had less injury and higher proliferative index(PI) in group A than in group B. The hyperplasiaof granulation tissue and vascular endothelial and the expression of bFGF were more evident in group A. After 5 days, the proliferation of endothelial cells and granulation and the protein synthesis of fibroblasts were more active in groupA, and at this moment, fresh collagen appeared and proliferated more actively in group B. After 7-14 days, epidermic structure and dermic microvascular density became normal gradually, the granulation on grafting base matured and transformed into fibrous connective tissue in group A. The same change deferred about 2 days in group B. After 21 days, the above pathologic change in group A was less than that in group B. After 3060 days of operation, Group A achieved much less contraction and transfiguration than Group B, and the grafted skin was tender and movable. Conclusion Autoskin grafting in fault hypodermis wound of granulation excision has a better effect than traditional operation.

        • THE EFFECT OF COMBINED TREATMENT OF RECOMBINANT HUMAN GROWTH HORMONE AND INSULIN-LIKE GROWTH FACTOR-1 ON WOUND HEALING AND PROTEIN CATABOLISM IN BURNED RATS

          OBJECTIVE: To investigate the effect of combined treatment of recombinant human growth hormone (rhGH) and insulin-like growth factor-1 (IGF-1) on wound healing and protein catabolism in burned rats. METHODS: Forty Wistar rats with deep II degree scald injury were divided randomly into four groups and received rhGH (0.1 U/kg.d), rhGH (0.1 U/kg.d) plus IGF-1 (2.0 mg/kg.d), IGF-1 (2.0 mg/kg.d) and Ringer’s solution (2 ml/kg.d, as control group) respectively. The wound healing time and protein catabolism levels of every groups were compared after 2 weeks. RESULTS: Total body weight began to increase after 2 weeks in rhGH group and rhGH plus IGF-1 group, but in control group, it was occurred after 4-5 weeks. The body weight of rhGH plus IGF-1 group was 1.65 times than that of rhGH group. The wound healing time in rhGH plus IGF-1 group (17.1 +/- 4.4) days was significantly lower than that of rhGH (20.5 +/- 4.8) days and control group (29.7 +/- 6.3) days. The protein level of rhGH plus IGF-1 group was significantly higher than that of control group and rhGH group. CONCLUSION: It suggests that rhGH plus IGF-1 with synergism is more effective in promoting wound healing and increasing the protein catabolism.

        • Development of Animal Burn Models in Rats: A Systematic Review

          ObjectiveTo assess the methods, processes and evaluation criteria of burn models in rats. MethodsDatabases including MEDLINE, EMbase, CBM, CNKI, VIP and WanFang Data were electronically searched from inception to February 2016. The animal studies of burn models development in rats were included. Two reviewers independently screened literatures, extracted data, assessed the quality of included studies, and analyzed the outcomes. ResultsFifteen studies including 38 burn groups were identified. The results indicated:(1) depth of burn:50.00% burn models were partial thickness burn (II°), and 21.05% were full thickness burn (III°). (2) methods of induction:80.00% burn models were developed through high temperature liquid or solid tools. (3) burn sites:93.33% burns were on the back of rats. (4) induction temperature:66.67% induction temperature was between 80℃ and 100℃. (5) induction time:86.67% induction time was between 3 seconds and 100 seconds. (6) anesthesia:40.00% rats were anesthetized with intraperitoneal injection of pentobarbital sodium. (7) skin preparation:60.00% studies reported the skin was prepared by physical and chemical methods. (8) housing post surgery:13.33% studies reported the housing temperature post surgery. (9) intervention post surgery:13.33% studies reported antishock treatment. (10) assessment criteria:almost all studies evaluated the depth of burn through the macroscopic and microcosmic assessment. ConclusionBased on current animal studies, most burn models in rats are partial thickness and full thickness burn on the back of rats. Burn is inducted mainly by 80℃ to 100℃ hot liquid or solid tools within 30 seconds, according with skin preparation, anesthesia, antishock or analgesia management. The depth of burn is evaluated by the macroscopic and microcosmic assessment. However, there is no standard for the methods, processes, assessment and reporting of development of burn model in rats.
