        • Original Study of A Long-term Aortic Valve Pump of 23mm Outer Diameter, Weighing 31g

          Objective To investigate the feasibility of a long-term left ventricular assist device placed in the aortic valve annulus for terminal cardiopathy. Methods An implantable aortic valve pump (23ram outer diameter, weighing 31g) was developed. There were a central rotor and a stator in the device. The rotor was consisted of driven magnets and an impeller, the stator was consisted of a motor coil with an iron core and outflow guide vanes. The device was implanted identical to an aortic valve replacement, occupying no additional anatomic space. The blood was delivered directly from left ventricle to the aortic root by aortic valve pump like natural ventricle, neither connecting conduits nor "bypass" circuits were necessary, therefore physiologic disturbances of natural circulation was less. Results Aortic valve pump was designed to cycle between a peak flow and zero net flow to approximate systole and diastole. Bench testing indicated that a blood flow of 7L/min with 50 mmHg(1kPa = 7.5mmHg) pressure could be produced by aortic valve pump at 15 000r/min. A diastole aortic pressure of 80mmHg could be maintained by aortic valve pump at 0L/min and the same rotating speed. Conclusions This paper exhibits the possibility that an aortic valve pump with sufficient hemodynamic capacity could be made in 23mm outer diameter, 31g and it could be implantable. This achievement is a great progress to extend the applications of aortic valve pump in clinic and finally in replacing the natural donor heart for heart transplantation. Meanwhile, this is only a little step, because many important problems, such as blood compatibility and durability, require further investigation.

        • Neurofeedback technology based on functional near infrared spectroscopy imaging and its applications

          Neurofeedback (NF) technology based on electroencephalogram (EEG) data or functional magnetic resonance imaging (fMRI) has been widely studied and applied. In contrast, functional near infrared spectroscopy (fNIRS) has become a new technique in NF research in recent years. fNIRS is a neuroimaging technology based on hemodynamics, which has the advantages of low cost, good portability and high spatial resolution, and is more suitable for use in natural environments. At present, there is a lack of comprehensive review on fNIRS-NF technology (fNIRS-NF) in China. In order to provide a reference for the research of fNIRS-NF technology, this paper first describes the principle, key technologies and applications of fNIRS-NF, and focuses on the application of fNIRS-NF. Finally, the future development trend of fNIRS-NF is prospected and summarized. In conclusion, this paper summarizes fNIRS-NF technology and its application, and concludes that fNIRS-NF technology has potential practicability in neurological diseases and related fields. fNIRS can be used as a good method for NF training. This paper is expected to provide reference information for the development of fNIRS-NF technology.

        • The Effective Orifice Area and Haemodynamics of 19mm St. Jude Regent Valve——An Early in Vivo Study

          Abstract: Objective To investigate the in vivo effective orifice area (EOA) and whether prosthesispatient mismatch (PPM) is going to happen after 19mm St.Jude Regent valves replacement were performed. Methods Twentythree patients with valvular heart diseases were divided into 2 groups according to aortic annular diameter. 19mm St. Jude Regent valves were replaced in aortic valve place (Regent valve group), and 21mm other doubleleaflet mechanical valves were replaced in aortic place (other valves group). All of the operations were accomplished under cardiopulmonary bypass in West China Hospital. All of the patients were followed up in 3 to 6 months after their surgery processes. Color Doppler echocardiography was used to measure the hemodynamic parameters in their followup. Then left ventricular mass (LV mass), EOA and effective orifice area index (EOAI) etc. were calculated and compared by using SPSS 12.0. Results No cardiac episodes were detected during the perioperative period and 36 months after operation. LVmass reduced in 36 months after operation in both groups. No statistical significance of EOA, EOAI and LVmass reduction were detected between the two groups. Conclusions (1)The EOA and in vivo hemodynamic effects of 19mm Regent valve are similar to 21mm other double leaflet mechanical valve. (2)It is safe to use 19mm Regent valve in those patients whose aortic annular diameter are small and need double valve replacement. Using 19mm Regent valve will not cause short time cardiac episodes and PPM. (3)More further works should be done to make sure what is the standard of PPM in Chinese people, such as enlarging the patients number and prolonging the followup time. (4) Further clinical and followup works should be done to make St.Jude Regent valve’s feature out.

        • Effects of Different Levels of Intra-Abdominal Pressure on Hemodynamics and Oxygen Metabolism in Pig

          Objective To study the effects of different levels of intra-abdominal pressure (IAP) on hemodynamics and oxygen metabolism in pig and to find the parameters early reflecting hemodynamics and oxygen metabolism of intra-abdominal hypertension (IAH). Methods Fifteen pigs were anaesthetized, and trachea intubation and transfemoral cannula were performed then. Swan-Ganz catheters were inserted to pulmonary artery via internal jugular vein, and aeroperitoneum was set up by airing CO2. These pigs were randomly divided into three groups (5 pigs in each group) according to different levels of IAP (IAP10, IAP20, IAP30). Hemodynamics and oxygen metabolism values were observed and recorded before airing, 6, 12, 18 and 24 hours after airing, respectively. Results There was no remarkable changes of hemodynamics and oxygen metabolism values in IAP10 group (Pgt;0.05); When the IAP level reached 20 cm H2O, there was significant elevation (P<0.01) in artery blood lacticacid (ABL), and mixed venous oxygen saturation (SvO2) decreased significantly (P<0.05) at 24 hours. When the IAP level reached 30 cm H2O, HR began to increase significantly at 18 hours after airing (P<0.05). After 24 hours, all the indices of hemodynamics and oxygen metabolism were either significantly increased or decreased in IAP30 group. Two pigs in the group of IAP30 died during 24 hours after airing, while other pigs survived. Conclusion This study demonstrates that the hemodynamics and oxygen metabolism values begin to change in pigs when the IAP level reaches 20 cm H2O, and high level of IAP that lasted for 24 hours shows deterioration of hemodynamic and oxygenation levels. The indices of SvO2 and ABL can early reflect tissue oxygenation in IAH.

        • Influence of bionic texture coronary stent on hemodynamics after implantation

          To explore the influence of bionic texture coronary stents on hemodynamics, a type of bioabsorbable polylactic acid coronary stents was designed, for which a finite element analysis method was used to carry out simulation analysis on blood flow field after the implantation of bionic texture stents with three different shapes (rectangle, triangle and trapezoid), thus revealing the influence of groove shape and size on hemodynamics, and identifying the optimal solution of bionic texture groove. The results showed that the influence of bionic texture grooves of different shapes and sizes on the lower wall shear stress region had a certain regularity. Specifically, the improvement effect of grooves above 0.06 mm on blood flow characteristics was poor, and the effect of grooves below 0.06 mm was good. Furthermore, the smaller the size is, the better the improvement effect is, and the 0.02 mm triangular groove had the best improvement effect. Based on the results of this study, it is expected that bionic texture stents have provided a new method for reducing in-stent restenosis.

        • 局部應用噻嗎心安對人眼脈絡膜血液循環的影響

          The effcet of topical timolol maleate 0.5% on the coroidal circulation was investigaed in 22 normal subjects using the color Doppler,with one eyes as the timolo-treated eye and the contralateral eye as the auto-control eye in each individual.The result showed that,of the 2 groups of eyes,there was a sighificant increase in time average maximum velocity(TAMX),systolic peak velocity(Vs),and distolic velocity(Vd) in timolol-treated eyes.The resistance index in timolol-treated eyes was lower than in the control eyes(p<0.05). Significant linear correlation in the timolol-treated eyesfound between perfusion and TAMX.It is thought that the increase in volumetric blood flow rate by timolol is due to two concurrent processes:1.an indrect effect on the choroidal circulation through the increase in perfusion pressure; 2.a dirct effect on choroidal vessels resulting from action of drug on adrenergic receptors that is present in the choroidal vasculature.         • Intraoperative Monitor and Modulation of Portal Vein Hemodynamics During Living Donor Liver Transplantation to Prevent Small-for-Size-Syndrome 

          Objective To analyze the effect of monitoring and modulating the portal vein pressure and blood flow during living donor liver transplantation (LDLT) on preventing small-for-size-syndrome (SFSS). Methods Data of forty-four LDLT recipients between Oct.2007 and Oct.2008 were reviewed. Actual graft-to-recipient weight ratio(GRWR), portal vein flow and pressure during operation and syndrome of SFSS after operation were recorded. The patients received splenectomy or splenic artery ligation according to actual GRWR, portal vein flow and pressure and WBC. Relationships between patients’ GRWR, portal vein flow, portal vein pressure and occurrence of SFSS were analyzed. Results Six patients received splenectomy and 7 patients received splenic artery ligation to decrease the portal vein flow and pressure during the operation. The portal vein flow and pressure decreased after splenectomy (Plt;0.05). The portal vein pressure decreased (Plt;0.05) and the portal vein flow had no significant change after splenic artery ligation (P>0.05). No SFSS occurred after operation. Conclusion Modulation of portal vein flow and pressure by splenectomy or splenic artery ligation during LDLT operation can decrease the portal vein flow and pressure, and which can prevent the incidence of SFSS.

        • MECHANICAL AND HEMODYNAMIC CHANGES IN AUTOGENOUS VEIN GRAFTS

          This study was performed on canine femoral veins which were interpositionally implanted into the femoral arteries and the investigation was in terms of zero-stress state, compliance and hemodynamic assessment. The results revealed that the vein grafts had the similar characteristics of compliance with the normal veins. Using Doppler ultrasonography to monitor the blood flow velocity through the vein grafts, the hemodynamic parameters such as pulsatility index (PI) and blood flow volume were evaluated consecutively within one month after the operations .No significant differences were found between these parameters at different time points. It was suggested that autogenous vein graft had an adaptive course when operating in an arterial hemodynamic circumstances and It’s mechanical changes did not bear upon the hemodynamics through the vein graft.

        • Effect of a delay mode of a ventricular assist device on hemodynamics of the cardiovascular system

          The implantation of biventricular assist device (BiVAD) is more challenging than that of left ventricular assist device for the interaction in the process of multiple input and output. Besides, ventricular assist device (VAD) often runs in constant speed (CS) mode in clinical use and thus BiVAD also faces the problems of low pulsation and imbalance of blood volume between systemic circulation and pulmonary circulation. In this paper, a delay assist mode for a VAD by shortening the support time of VAD was put forward. Then, the effect of the delay mode on cardiac output, pulsation and the function of the aortic valve was observed by numerical method and the rules of hemodynamics were revealed. The research showed that compared with VAD supported in CS mode, the VAD using delay mode in systolic and diastolic period proposed in this paper could meet the demand of cardiac output perfusion and restore the function of the arterial valves. The open ratio of aortic valve (AV) and pulmonary valve (PV) increased with the time set in delay mode, and the blood through the AV/PV helped to balance the left and the right cardiac volume. Besides, delay mode also improved the pulsation index of arterial blood flow, which is conducive to the recovery of the ventricular pulse function of patients.
