        • Establishment and validation of risk prediction model for prolonged mechanical ventilation after lung transplantation

          ObjectiveProlonged mechanical ventilation (PMV) is a prognostic marker for short-term adverse outcomes in patients after lung transplantation.The risk of prolonged mechanical ventilation after lung transplantation is still not clear. The study to identify the risk factors of prolonged mechanical ventilation (PMV) after lung transplantation.Methods This retrospective observational study recruited patients who underwent lung transplantation in Wuxi People’s Hospital from January 2020 to December 2022. Relevant information was collected from patients and donors, including recipient data (gender, age, BMI, blood type, comorbidities), donor data (age, BMI, time of endotracheal intubation, oxygenation index, history of smoking, and any comorbidity with multidrug-resistant bacterial infections), and surgical data (surgical mode, incision type, operation time, cold ischemia time of the donor lung, intraoperative bleeding, and ECMO support), and postoperative data (multi-resistant bacterial lung infection, multi-resistant bacterial bloodstream infection, and mean arterial pressure on postoperative admission to the monitoring unit). Patients with a duration of mechanical ventilation ≤72 hours were allocated to the non-prolonged mechanical ventilation group, and patients with a duration of mechanical ventilation>72 hours were allocated to the prolonged mechanical ventilation group. LASSO regression analysis was applied to screen risk factors., and a clinical prediction model for the risk of prolonged mechanical ventilation after lung.ResultsPatients who met the inclusion criteria were divided into the training set and the validation set. There were 307 cases in the training set group and 138 cases in the validation set group. The basic characteristics of the training set and the validation set were compared. There were statistically significant differences in the recipient’s BMI, donor’s gender, CRKP of the donor lung swab, whether the recipient had pulmonary infection before the operation, the type of transplantation, the cold ischemia time of the donor lung, whether ECMO was used during the operation, the duration of ECMO assistance, CRKP of sputum, and the CRE index of the recipient's anal test (P<0.05). 2. The results of the multivariate logistic regression model showed that female recipients, preoperative mechanical ventilation in recipients, preoperative pulmonary infection in recipients, intraoperative application of ECMO, and the detection of multi-drug resistant Acinetobacter baumannii, multi-drug resistant Klebsiella pneumoniae and maltoclomonas aeruginosa in postoperative sputum were independent risk factors for prolonged mechanical ventilation after lung transplantation. The AUC of the clinical prediction model in the training set and the validation set was 0.838 and 0.828 respectively, suggesting that the prediction model has good discrimination. In the decision curves of the training set and the validation set, the threshold probabilities of the curves in the range of 0.05-0.98 and 0.02-0.85 were higher than the two extreme lines, indicating that the model has certain clinical validity.ConclusionsFemale patients, Preoperative pulmonary infection, preoperative mechanical ventilation,blood type B, blood type O, application of ECMO assistance, multi-resistant Acinetobacter baumannii infection, multi-resistant Klebsiella pneumoniae infection, and multi-resistant Stenotrophomonas maltophilia infection are independent risk factors for PMV (prolonged mechanical ventilation) after lung transplantation.

        • Promotion and experience of quality control system for lung transplantation in China

          [Abstract]It is an effective way of constructing a lung transplantation quality control system suitable for China's national conditions to break through the many dilemmas in China. Under the leadership of the National Quality Control Center, a stage-by-stage and full-scale quality control system for lung transplantation in China has been gradually constructed and extended to many lung transplantation centers nationwide, which has strongly promoted the development of lung transplantation in China. This article outlines the construction, promotion and experience of China's lung transplantation quality control system, aiming to provide reference for further development of relevant measures to promote the homogenization of lung transplantation in China.

        • Effect of Astilbin on Lung Allograft Rejection in Rats’ Transplantation Model

          Objective To investigate the suppression effect and mechanism of Astilbin on lung allograft rejection in rats, in order to know the function of Astilbin on rats’ lung acute rejection. Methods The model of rat left lung transplantation was set up. Sixty lung transplanted rats were divided into two groups randomly, control group: rats were fed with normal saline 1ml per day, experimental group: rats were fed with Astilbin 1mg/kg per day. Survival time, transforming rate of T cells in spleen, activity of interleukin 2 (IL-2) in spleen lymph cells and apoptosis of T cells were observed. Changes in ultrastructure of pulmonary arteries were observed by electron microscope. Results The survival time in experimental group was prolonged than that in control group (25.4±2.1 d vs. 13.4±1.2 d;t=2.042, Plt;0.05). Transforming rate of T cells of spleen in experimental group was significant lower than that in control group (23 465.8±8 783.4 cpm vs. 74 567.3±12 874.6 cpm; t=2.284,Plt;0.05).Activity of IL-2 of spleen lymph cells in experimental group was significant lower than that in control group (425±2.65U/ml vs. 23.46±1.82U/ml; t=3.165, Plt;0.01).Effectively derive apoptosis of activated T cells in acute rejection were observed in experimental group, the ultrastructure of pulmonary arteries showed attenuated injury in experimental group. Conclusion Astilbin decreased the IL-2 concentration in plasma and induced the apoptosis in activated T cells, then suppressed the acute rejection of lung allograft and prolonged the survival period of lung transplantation rats.

        • Pneumocystis jirovecii pneumonia diagnosed by next-genetation sequencing after lung transplantation: case report and literature review

          ObjectiveTo explore the clinical value of next-generation sequencing (NGS) in the diagnosis of Pneumocystis jirovecii pneumonia (PCP).MethodsTwo patients with Pneumocystis jirovecii pneumonia after lung transplantation were detected by NGS in the sputum and bronchoalveolar lavage fluid. The clinical data, imaging features, laboratory examination and treatment of the two patients were reported. A systematic literature review was performed for similar published cases in PubMed database, using the keywords "lung transplantation/solid organ transplantation" and "Pneumocystis jirovecii".ResultsThere were six references based on the keywords of "lung transplantation " and " Pneumocystis jirovecii ", of which three were case report. Sixty-six lung transplant patients were complicated with Pneumocystis jirovecii in total. The clinical manifestations of Pneumocystis jirovecii pneumonia were fever and dyspnea of different degrees. The diffuse "ground glass" infiltration could be shown on imaging. Computer tomography scan of chest was a sensitive method to detect PCP. Combined immunofluorescence microscope/PCR/serum 3-β-D-glucan could effectively improve the accuracy of microbiology detection. In addition, NGS could quickly and accurately identify pathogenic bacteria, give guidance for treatment and improve prognosis so as to benefit patients well. Trimethoprim/sulfamethoxazole (TMP/SMZ) was the preferred choice for the treatment of PCP patients.ConclusionsPneumocystis jirovecii pneumonia is more common in patients with immunodeficiency or immunosuppression. NGS can help rapid and accurate diagnosis, and the treatment should be early and sufficient.

        • Visualization analysis of hotspots and trends of lung transplantation nursing research based on CiteSpace

          Objective To explore the research hotspots and latest progress of lung transplantation nursing at home and abroad, and to provide reference ideas and directions for the future related research. Methods Through the collection of CNKI and Web of Science core database, the related literature of lung transplantation nursing from inception to December 2023 was retrieved and CiteSpace software was used to focus on keyword co-occurrence, clustering and emergence. Results A total of 352 Chinese literature and 126 English literature were included in this study, and the number of papers published at home and abroad showed an overall upward trend. There were 34 core authors of Chinese literature and 83 core authors of English literature. The core authors led the team to promote research and promote cooperation at home and abroad. At present, the cooperation among research teams needed to be strengthened. Chinese literature keywords included nursing, lung transplantation, extracorporeal membrane oxygenation, lung rehabilitation, lung transplantation surgery, complications. English literature keywords included lung transplanation, quality of life, international society, disease, bone marrow transplantation. There were 15 clusters and 25 emergent keywords in the CNKI database, and 12 clusters and 25 emergent keywords in the Web of Science core database. ConclusionThe study of lung transplantation nursing at home and abroad has been paid more and more attention, and the research is getting more and more sophisticated. The future study can realize the holistic care of patients during perioperative period on the basis of clinical diagnosis and nursing technology, and improve the survival of patients after operation.

        • The Clinical Analysis of 65 Cases with Lung Transplantation

          Objective To investigate the experience of operative technique of donor organ harvesting and lung transplantation in some unusual circumstance, and to improve surgical success ratio of lung transplantation. Methods Lung transplants were preformed in 65 cases, including 47 singlelung transplants and 18 double single lung transplants. All the recipients were suffered from intensive respiratory failure,and nine patients were longterm ventilatordependented of the total. The recipients included emphysema (n=23), pulmonary fibrosis (n=24), pneumosilicosis(n=5), pulmonary tuberculosis(n=2), lymphangioleiomyomatosis(n=1) and ventricular septal defect(VSD) or VSD with Eisenmenger’s syndrome(n=4),bronchiectasis (n=4), diffuse panbronchiolitis (n=1) and primary pulmonary hypertension(n=1). Retrospectively summarize clinical experience of lung transplant operation especially experience of dealing with special circumstances encountered in operation. Results 64 donor organ harvesting were achieved successfully. Inhospital death was 11cases (16.9%) after operation. Early death was due to primary lung graft dysfunction (n=3), severe infection(n=6), acute rejection(n=1), pulmonary vein embolism(n=1). Complications took place after operation in 9 cases, to exploratory thoracotomy to stop bleeding after transplantation in 3 cases, pulmonary artery anastomosis again because of stenosis in 1 case, bronchus stoma stenosis in 3 cases, pulmonary infarction in 2 cases, of which one patient accepted pulmonary lobectomy. Follow-up period was from 1.0 year to 5.6 years of 54 cases. 1year survival rate was 72.3%(47/65).The pulmonary function was improved and the quality of life is well in most patients of the group. Conclusion To improve the technique of donor organ harvesting and lung transplantation is important to decrease the early mortality after transplantation.

        • Research status of lung transplantation and standardized management during transplantation period

          Lung transplantation is the only treatment for patients with end-stage lung diseases. And this field is also a research hotspot in the international field at presen. Relevant researches not only promote the progress and development of lung transplantation, but also improve the life quality of patients after transplantation. With the development of lung transplantation technology, the guidelines for identifying candidates are constantly being revised, and the source of donor lungs has always been an urgent problem for all transplantation centers. The standardized management during transplantation period involves anesthesia induction, intraoperative fluid management, airway management, management of important operative steps and postoperative pain management. Extracorporeal life support (ECLS) includes cardiopulmonary bypass (CPB) and extracorporeal membrane oxygenation (ECMO). With the progress and development of ECLS technology, the advantages of ECMO as a bridge for lung transplantation, intraoperative and postoperative circulatory support are becoming more and more prominent, enabling recipient patients to successfully pass the period of lung transplantation. Although lung transplantation in basic science and clinical researches has got a lot of progress, to improve the survival rate after transplantation, we must overcome many challenges including how to successfully perform lung transplantation, expand lung donor library, induce tolerance, and prevent complications after transplantation, primary graft dysfunction (PGD), cell and antibody mediated rejection and infection.
