        • Application study of a novel locking anatomical plate in treatment of Neer type Ⅱb and type Ⅴ distal clavicle fractures

ObjectiveTo evaluate the feasibility and effectiveness of a novel locking plate designed for the acromial end of the clavicle (hereinafter referred to as the “novel locking plate”) in the treatment of Neer type Ⅱb and type Ⅴ distal clavicle fractures. Methods Between February 2024 and January 2025, a total of 12 patients with Neer type Ⅱb and type Ⅴ distal clavicle fractures resulting from trauma were treated using the novel locking plate. The cohort comprised 7 males and 5 females, with ages ranging from 22 to 65 years (mean, 49.6 years). According to the Neer fracture classification system, 9 cases were classified as type Ⅱb and 3 cases as type Ⅴ. The interval from injury to operation ranged from 2 to 6 days (mean, 3.9 days). After achieving anatomical reduction of the fracture, the novel locking plate was applied for internal fixation, accompanied by anatomical reconstruction of the coracoclavicular ligament. Operative time, intraoperative blood loss, and incision healing were meticulously documented. During follow-up period, the radiographic examinations were taken to assess fracture healing, and was evaluated using the Constant-Murley score criteria was used to evaluate the shoulder joint function. ResultsAll 12 operations were completed. The operative time ranged from 55 to 90 minutes (mean, 74 minutes), and intraoperative blood loss ranged from 50 to 100 mL (mean, 85 mL). All incisions healed by first intention. All patients were followed up 6-18 months (mean, 11.6 months). At last follow-up, the radiographic examination demonstrated that all fractures had achieved bony union; the Constant-Murley score ranged from 92 to 96 (mean, 94.3), all rated as excellent. ConclusionFor Neer type Ⅱb and type Ⅴ distal clavicle fractures, the combination of the novel locking plate and anatomical reconstruction of the coracoclavicular ligament provides reliable internal fixation, ensures satisfactory fracture reduction, promotes fracture healing, and yields highly satisfactory effectiveness.

        • The prognostic value of ezrin expression in bone and soft tissue sarcomas: a meta-analysis

ObjectiveTo systematically review the predictive value of ezrin expression in bone and soft tissue sarcomas.MethodsDatabases including PubMed, EMbase, The Cochrane Library (Issue 2, 2016), CNKI, CBM, VIP, and WanFang Data were searched to collect cohort studies about the prognostic value of ezrin expression in bone and soft tissue sarcomas from inception to May 2016. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Then meta-analysis was performed by using RevMan 5.3 software.ResultsFinally, a total of 17 cohort studies, involving 1 460 patients were included. The results of meta-analysis showed that: the overall survival (HR=1.90, 95%CI 1.70 to 2.13, P<0.000 01), event-free survival (HR=2.42, 95%CI 1.53 to 3.84, P=0.0002), metastasis-free survival (HR=2.09, 95%CI 1.10 to 3.97, P=0.02) in the bone and soft tissue sarcomas patients with ezrin high expression were lower than patients with lower expression. The same results were also observed in subgroup analysis according to histologica type and ethnicity in overall survival.ConclusionEzrin high expression may be used as a predictive maker for poor prognosis in bone and soft tissue sarcomas. For the quantity and quality limitation of the included studies, this conclusion still needs to be further proved by performing more high quality studies.

        • Research progress on bone repair biomaterials with the function of recruiting endogenous mesenchymal stem cells

Objective To review the research progress on bone repair biomaterials with the function of recruiting endogenous mesenchymal stem cells (MSCs). Methods An extensive review of the relevant literature on bone repair biomaterials, particularly those designed to recruit endogenous MSCs, was conducted, encompassing both domestic and international studies from recent years. The construction methods and optimization strategies for these biomaterials were summarized. Additionally, future research directions and focal points concerning this material were proposed. Results With the advancement of tissue engineering technology, bone repair biomaterials have increasingly emerged as an ideal solution for addressing bone defects. MSCs serve as the most critical “seed cells” in bone tissue engineering. Historically, both MSCs and their derived exosomes have been utilized in bone repair biomaterials; however, challenges such as limited sources of MSCs and exosomes, low survival rates, and various other issues have persisted. To address these challenges, researchers are combining growth factors, bioactive peptides, specific aptamers, and other substances with biomaterials to develop constructs that facilitate stem cell recruitment. By optimizing mechanical properties, promoting vascular regeneration, and regulating the microenvironment, it is possible to create effective bone repair biomaterials that enhance stem cell recruitment. Conclusion In comparison to cytokines, phages, and metal ions, bioactive peptides and aptamers obtained through screening exhibit more specific and targeted recruitment functions. Future development directions for bone repair biomaterials will involve the modification of peptides and aptamers with targeted recruitment capabilities in biological materials, as well as the optimization of the mechanical properties of these materials to enhance vascular regeneration and adjust the microenvironment.
