Health poverty alleviation is an important practice in implementing the basic strategy of precision poverty alleviation. It is also an important measure to win the battle against poverty in rural areas. Through the investigation of local medical and health conditions, West China Hospital of Sichuan University built a demand-oriented framework for medical poverty alleviation in Ganzi Prefecture, and gradually carried out precision top-level design, discipline construction, technical training, talent training, endemic disease prevention and control; through establishinga regional medical cooperation network, built featured specialies, built the backbone of medical and health forces, achieved the prevention of endemic diseases throughout life cycle, and improved the overall medical and health service capacity of Ganzi Prefecture. This article introduces the exploration and practice of the demand-oriented model in Ganzi Prefecture by West China Hospital of Sichuan University, aiming at providing a guide for the precision health poverty alleviation in China.
West China Hospital of Sichuan University as a national-level regional medical center in the western part of the country, focused on the actual situation in Tibet and actively carried out precision health poverty alleviation work. Guided by " precision”, the hospital has built a close-knit medical association – Hospital of Tibet People’s Government in Chengdu Office, and through the comprehensive improvement of medical care, teaching, scientific research, and management, creates a medical and health service system with Tibet characteristics. Combining " blood transfusion” and " hematopoietic” to build a " West China Model” for precision health poverty alleviation, West China Hospital of Sichuan University fully demonstrates the public welfare and responsibility of a national-level regional medical center, and constantly exerts regional radiation and leading role, promotes the medical and health service system continuous improvement in Tibet.
Objective To survey the current status of residents’ health examination in township medical units in Shuangliu county, so as to provide references for exploring the residents’ health examination mode suitable for the well-off rural hospitals, and for improving the effects of physical examination. Methods The self-designed questionnaires were made to survey the current situation of health examination for the residents in 24 township medical units in Shuangliu County, including 3 central township hospitals, 3 community health centers and 18 general township hospitals; and the outline was designed to interview the people responsible for managing and implementing the resident’s examination work. The Epidate 3.1 was used for data input, the SPSS 17.0 was used for descriptive statistics and stratified analysis, and the classification method was adopted for qualitative research of personal interview data. Results The number of primary health care units for carrying out residents’ health examination got gradually increased year by year from 2008 to 2010; and all 24 township-level medical units had already carried out that examination by 2010. But some issues still existed such as lower participation rate and ineffective utilization of the archives. Conclusion a) It is necessary to prepare well, enhance organizing, expand publicity, and increase participation rate when primary health care units carry out the health examination for residents; b) It is necessary requires to fasten the informatization construction of health archives, and to quicken the process and analysis of examination information; and c) It is advisable to constantly optimize the examination items according to exam information, to improve the qualification of medical staffs, and to fasten the construction of exam team.
Coronavirus disease 2019 epidemic is a serious and infectious public health event that causes physical damage and may also have an impact on mental health. West China Hospital of Sichuan University combines the internet hospital platform with the traditional telephone model, establishes a new coronavirus disease 2019 intervention integration platform, and builds a new “four-in-one” medical service system, which contains epidemic consultation, psychological consultation, self-evaluation and home prevention and control. It has established a way for the public to communicate with professional, reduced the spread of coronavirus disease 2019 in space, and effectively carried out psychological decompression and comfort work, disseminated the knowledge of fighting coronavirus disease 2019, and provided medical advice. This article mainly introduces the out-of-hospital psychological intervention model of West China Hospital of Sichuan University during the coronavirus disease 2019 epidemic, and aims to provide a reference for exploring the psychological intervention methods for major public health emergencies.
Cadre education and training is the elementary step in building a high quality, basic, and strategic cadre ranks project. The summer training of cadres in West China Hospital of Sichuan University is one aspect of the training system of cadres in our hospital. This paper introduces the current situation of summer cadre training, the training needs of cadres and the changes of internal and external environment in our hospital from 2009 to 2012, so as to continue constructing our summer cadres training product brand.
ObjectivesTo analyze epidemiological characteristics of leprosy in Sichuan province from 2000 to 2017.MethodsCase data of all new leprosy patients in Sichuan province from 2000 to 2017 were collected. A retrospective analysis of its epidemiological characteristics was performed by using SPSS 19.0 software.ResultsA total of 3 208 cases of leprosy were detected during 2000 to 2017, of whom 2 197 (71.28%) were male, 885 (28.72%) were female. The younger cases whose ages were less than 14 were 82 (2.66%), and the cases with grade 2 disabilities were 614 (19.92%). The mean age of male was older than female (41.64±14.26 vs. 38.89±15.12 years, P<0.05). The grade 2 disability rate of male was significantly higher than that of female (20.94% vs. 17.40%, P<0.05). Self-report was the most common method of discovery. But the ratio of male who were detected through contact examination was significantly lower than that detected through dermatological clinic, self-report, clues check and report (the ratio of male to female was 1.57, 2.38, 2.88, 2.48, 2.37, respectively, P<0.05).ConclusionsThe case detection of leprosy declines annually in Sichuan province from 2000 to 2017, especially in high-endemic area and male patients. Female patients are younger than male patients when they are detected. The grade 2 disability situation of male patients is significantly more serious than that of female patients. Self-report is the most common way of discovery, while women are more passive.
Objective To evaluate the quality of randomized controlled trials in otorhinolaryngology in China and provide to comprehend the possibility of its contribution in providing reliable, evidence in clinical practice; thus providing evidence to elevate the clinical treatment level. Methods Five Chinese clinical otorhinolaryngology journals were searched and randomized controlled trials were identified and analyzed according to the standards of evidence-based medicine. Results Two hundred and eighty seven issues were referred to and eighty-one randomized controlled trials were identified and analyzed. Of these randomized controlled trials, 34.57% (28/81) had definite diagnostic standards, 38.27% (31/81) had inclusion standards and 33.33% (27/81) had exclusion standards; only 1.23% (1/81) got the approval of the participants; 40.74% (33/81) had moderate sample size; 3.70% (3/81) had large sample size and no one mentioned sample size estimation; 81.48% (66/81) didn’t report the method of randomization and 38.27% (31/81) had baseline comparison; 18.52% (15/81) didn’t define the control interventions and 8.64% (7/81) even didn’t explicate the experimental interventions; 32.10% (26/81) used blank comparison; 86.42% (70/81) didn’t use blindness; 37.04% (30/81) didn’t mention the adverse effects; 23.46% (19/81) used accredited standards to evaluate the outcomes; l l.11% (9/81) mentioned the loss of participants and only 1.23% (1/81) treated the loss with statistics methods. Conclusions The quantity and quality of the otorbinolaryngologic randomized controlled trials in present review can not meet the clinical need. Higher quality of randomized controlled trials are required to improve the level of prevention and the treatment of otorhinolaryngologic diseases.
Objective
To comparatively analyze the rationality of emergency medication after Wenchuan earthquake with that after Lushan earthquake in West China Hospital of Sichuan University, based on the use of medicine of the victim’s disease spectrum.
Method
By using Excel, defined daily dose system (DDDs) and drug utilization index (DUI) were used as the evaluation indexes to analyze the rationality of emergency medication in West China Hospital of Sichuan University within one month after Wenchuan earthquake and Lushan earthquake.
Results
Within one month after Wenchuan and Lushan earthquake, there were 1 839 and 488 victims treated in the hospital, respectively. Within one month after the two earthquakes, the variation tendency of DDDs of drugs and number of victims was consistent, and the consistency was better in Lushan earthquake than that in Wenchuan earthquake. Among the 60 drugs which DDDs were ranked top five in their pharmacological class (top ten for antimicrobials) in Wenchuan earthquake, the majority of them were injections (injections vs. non-injections: 70.0% vs. 30.0%); the results showed that the medication (DUI=1) only accounted for 10.0%, the medication (DUI<1) accounted for 28.3%, which implied that the use of drugs was insufficient, the medication (DUI>1) accounted for 61.7%, which implied that drugs were overused; the average of DUI was 1.61. And in Lushan earthquake, injections also accounted for a larger proportion than non-injections (63.3%vs. 36.7%); the results showed that the medication (DUI=1) accounted for 15.0%, the medication (DUI<1) accounted for 38.3%, the medication (DUI>1) accounted for 46.7%; the average of DUI was 1.30.
Conclusions
Base on the DUI, we draw the conclusion that the rationality of emergency drug use and the timeliness of emergency drug supply were better in Lushan earthquake than those in Wenchuan earthquake. But the rationality of using the DUI, which is an evaluation index for normal conditions, to evaluate the emergency conditions still needs to be further verified.
Objective?To summarize the experience of providing medical supplies for the medical rescue after the Wenchuan earthquake so as to provide information for similar tasks in the future. Methods?We collected and analyzed relevant data from the Medical Supply Group of the Headquarters for Earthquake Resistance and Disaster Relief. Results?① The Medical Supply Group of the Headquarters for Earthquake Resistance and Disaster Relief was set up immediately after the quake. Professional software for the medical supply information management was developed to ensure the completeness and timeliness of relevant data. ② Within the first 3 days after the quake, 12 460 medical device parts were delivered to the disaster area, which effectively ensured the availability of medical supplies for the “golden 72 hours”. ③ A great amount of donated material from other provinces and overseas also arrived. These donations reached a peak 2 weeks after the quake. A total of 428 411 drug products, 196 504 medical device parts and 5 999.4 tons of disinfection material were successfully delivered to most of the quake-stricken areas within 24 hours after its delivery to Sichuan province. ④ Three months after the quake, the remaining 193 831 drug products, 96 423 medical device parts and 2 000 tons of disinfection material were disposed appropriately. A regional medical supply warehouse was established to reserve 129 895 drug products, 74 018.4 medical device part and 231.28 tons of disinfection material. On the 115th day after the quake, a portion of these reserves were allocated to the quake-hit areas in Panzhihua, Liangshan, and Yunnan province. ⑤ Nearly 20 tons of drug products, 10 tons of medical devices and 724.54 tons of disinfection materials were properly destroyed due to expiration or disqualification. ⑥ Based on the data collected during the disaster relief work, a series of articles were published. Conclusion?The medical supply work for the medical rescue after the Wenchuan earthquake was efficient, well-organized, transparent, and open.
Objective To survey the drug distribution status in 24 township medical units in Shuangliu county, so as to provide references to explore the drug distribution mode for well-off township hospital. Methods An outline was made to interview the relevant workers who were in charge of the rural drug distribution management in all levels of the food and drug supervision bureaus of Sichuan province, Chengdu city or Shuangliu county, the township hospitals, the village clinics, and the drug distribution enterprises. At the same time, a self-designed questionnaire was distributed to 24 county township medical units (ie, three central township hospitals, three community health centers, and 18 general township hospitals). The Epidate3.1 was used for data-inputting, and the SPSS 17.0 was used for descriptive statistics and stratified analyses. Results At present, there was only one enterprise taking responsibility of the rural drug distribution in Shuangliu county since the strategy of enterprise direct-distribution under “two nets” supervision starting iShuangliu county of Sichuan province; Township medical units; Drug distribution; Status surveyn 2003, and there was no obvious difference in distribution before and after that strategy. The general satisfaction of 24 county township medical units with the distribution enterprise was only 50%, and the satisfaction with the price and the number of drug distribution was the lowest, accounting for 37.5% and 33.3%, respectively. Conclusion The enterprise direct-distribution under the “two nets” supervision for rural drug distribution makes the drug distribution market regular and gets good social effect, but there are still the same problems such as lack of marketization, low transparency, and high drug price, so it is necessary to introduce a new competition and management mechanism to promote the new breakthrough in rural drug distribution.
