        • Indications for transcatheter interventions in tricuspid regurgitation

          Tricuspid regurgitation (TR) is a common but often neglected valvular heart disease. Growing evidence has highlighted its association with poor prognosis and the complexity of its management. In recent years, transcatheter tricuspid valve intervention (TTVI) has emerged as a promising minimally invasive approach, with accumulating evidence supporting its safety and short-term efficacy. Transcatheter approaches offer a new therapeutic alternative for high-risk TR patients, but the clinical indications for TTVI are still being refined. This review summarizes recent advancements in TTVI technology, compares the indications for surgical and transcatheter therapies, and discusses the critical role of right heart function and pulmonary circulation in therapeutic decision-making.

        • An in vivo study of ultrasonic monitoring imaging of microwave ablation based on Nakagami statistic parameter

          This paper explored the feasibility of using ultrasonic Nakagami statistic parameter imaging to evaluate the thermal lesion induced by microwave ablation (MWA) in porcine models. In this paper, thermal lesions were induced in livers and kidneys in 5 swines using a clinical MWA system. During this treatment progress, ultrasonic radiofrequency (RF) data were collected. The dynamic changes of Nakagami parameter in the thermal lesion were calculated, and the ultrasonic B-mode images and Nakagami images were reconstructed simultaneously. The contrast-to-noise ratio (CNR) between the thermal lesion and the surrounding normal tissue was calculated over the MWA procedure. After MWA, a bright hyperechoic region appeared in the ultrasonic Nakagami image as an indicator of the thermal lesion and this bright spot enlarged with lesion development during MWA exposure. The mean value of Nakagami parameter in the liver and kidney increased from 0.78 and 0.79 before treatment to 0.91 and 0.92 after treatment, respectively. During MWA exposure, the mean values of CNR calculated from the Nakagami parameter increased from 0.49 to 1.13 in the porcine liver and increased from 0.51 to 0.85 in the kidney, which were both higher than those calculated from the B-mode images. This in vivo study on porcine models suggested that the ultrasonic Nakagami imaging may provide an alternative modality for monitoring MWA treatment.

        • Advances in pathogenesis and nutritional intervention of sarcopenia in esophageal cancer

          Sarcopenia is a syndrome characterized by progressive decline in skeletal muscle mass, strength, and function, with a high prevalence in patients with esophageal cancer. Due to tumor consumption, feeding difficulties, systemic inflammatory response, and treatment-related toxicities, patients with esophageal cancer are particularly susceptible to sarcopenia. Numerous studies have demonstrated that sarcopenia is closely associated with increased postoperative complications, prolonged hospital stay, decreased quality of life, and reduced overall survival in esophageal cancer patients. The pathogenesis of sarcopenia involves multifaceted interactions, including tumor-related inflammatory cytokine release, mitochondrial dysfunction, imbalance between protein synthesis and degradation, neuromuscular junction dysfunction, and inadequate nutritional intake. In recent years, nutritional intervention has garnered widespread attention as an important strategy for sarcopenia prevention and treatment. Preoperative nutritional risk assessment, perioperative individualized nutritional support, supplementation with specific immunonutrients, and multidisciplinary collaborative management models centered on prehabilitation have shown positive effects in improving nutritional status and mitigating muscle loss in esophageal cancer patients. This review summarizes the research progress on the epidemiological characteristics, pathogenesis, and nutritional intervention strategies of sarcopenia in esophageal cancer patients, aiming to provide reference for the identification, assessment, and management of sarcopenia in clinical practice.

        • Construction and validation of the nursing quality evaluation indicator system for patients with skin flap transplantation

          Objective To construct a nursing quality evaluation indicator system for patients with skin flap transplantation and to provide a reference for the evaluation and continuous improvement of nursing quality in patients with skin flap transplantation. Methods With the three-dimensional quality structure theory as the theoretical framework, the nursing quality evaluation indicator system for patients with skin flap transplantation and indicator weights were established through literature review, expert meeting, Delphi method, and analytic hierarchy process. From October to December 2025, clinical application was carried out in departments performing flap transplantation surgeries nationwide to verify the feasibility and practicality of the indicator system. Results Two rounds of expert consultations were conducted. The expert authority coefficients were 0.941 and 0.950, respectively. The mean importance scores of all indicators were >3.50, the variation coefficients were <0.25, and the full-score rates were >20.00%. Kendall’s coefficients of concordance were 0.122 and 0.153 (P<0.05), respectively. The final nursing quality evaluation indicator system for patients with skin flap transplantation included 3 primary indicators, 7 secondary indicators, and 18 tertiary indicators. A total of 50 departments participated in the preliminary clinical application. The Cronbach’s α coefficient of the indicator system was 0.821, the Spearman-Brown coefficient for split-half reliability was 0.773, the average content validity index of the indicator system was 0.99, and the universal content validity index was 0.83. The satisfaction questionnaire score was 21.86±2.26. Conclusion The nursing quality evaluation indicator system for patients with skin flap transplantation is feasible and practical, reflects the disease-specific characteristics of skin flap transplantation, and can be used for nursing quality management in patients with skin flap transplantation.

        • Perioperative efficacy and safety of da Vinci robot-assisted bronchial sleeve lobectomy

          ObjectiveThrough the perioperative outcome analysis of da Vinci robot-assisted sleeve lobectomy, to clarify its efficacy and safety.MethodsA retrospective analysis was performed on 10 patients with centrally located lung cancer undergoing robot-assisted sleeve lobectomy from March to December 2019 in our center, including 9 males and 1 female, aged 45-67 (55.0±8.9) years. Preoperative imaging and bronchoscopy showed central non-small cell lung cancer, involving the right upper lung in 3 patients, right lower lung in 2 patients, the left upper lung in 4 patients, and left lower lung in 1 patient. The operation time, Docking time, intraoperative blood loss volume, bronchial anastomosis time, number of dissected lymph nodes, drainage volume and postoperative hospital stay were analyzed.ResultsThe da Vinci robot-assisted bronchial sleeve lobectomy was completed smoothly on 10 patients. The operation time was 135-183 (157.8±14.3) min, Docking time 6-15 (10.0±2.9) min, intraoperative blood loss volume 55-250 (124.5±61.8) mL, bronchial anastomosis time 17-40 (27.7±7.3) min, the number of dissected lymph nodes 16-23 (19.7±2.8), the drainage volume 200-600 (348.0±148.4) mL and postoperative hospital stay 7-11 (8.7±1.6) d. All patients had no bronchopleural fistula, pulmonary infection or atelectasis, and there was no perioperative death. Postoperative pathological findings were all squamous cell carcinoma.ConclusionDa Vinci robot-assisted sleeve lobectomy is safe and effective.

        • Low-dose growth hormone and hypocaloric nutrition attenuate the protein-catabolic response after major operation

          Objective To determine the effects recombinant human growth hormone (GH) and hypocaloric nutrition on postoperative convalescence, we performed a placebo-controlled randomized double-blind trial in 18 patients after elective gastrectomy or colectomy. Methods The subjects received parenteral nutrition containing 20 calories/kg per day and 1 g protein/kg per day. Daily injections of drug or placebo were given during the first postoperative week. Result The nine control subjects lost 3.3 kg (5.9% of preoperative weight) and had a cumulative nitrogen loss of 32.6 ± 4.2 g nitrogen at eight days. The patients receiving GH lost significantly less weight (1.3 kg) and nitrogen loss was 7.3 ± 3.1 g at eight days (Plt;0.001). Kinetic studies demonstrated that anabolic effects of GH were associated with increased protein synthesis, and amino acid flus studies across the forearm revealed increased uptake of amino acid nitrogen in the GH-treated patients. Body composition analysis revealed that the patients receiving GH maintained their lean body mass despite the major surgical procedure. Conclusion We conclude that the postoperative catabolic response can be modified with GH and hypocaloric nutrition. The metabolic and physiologic effects should now be studied in a larger number of patients to determine if this approach can reduce morbidity, mortality, and length of hospital stay for surgical patients.

        • Clinical efficacy of paclitaxel and carboplatin with versus without bevacizumab in treatment of advanced non-small cell lung cancer: A systematic review and meta-analysis

          ObjectiveTo systematically evaluate the clinical efficacy and adverse reactions of paclitaxel and carboplatin with or without bevacizumab in the treatment of non-small cell lung cancer (NSCLC).MethodsThe databases including PubMed, The Cochrane Library, EMbase, CNKI, Wanfang Data, VIP and CBM were searched from inception to October 2022 to collect randomized controlled trials of the clinical efficacy of paclitaxel and carboplatin with or without bevacizumab for the treatment of NSCLC. RevMan 5.4 software was used for meta-analysis.ResultsEight randomized controlled trials were enrolled, involving a total of 1 724 patients. Meta-analysis showed that for the treatment of NSCLC, the disease control rate, overall response rate, 1-year survival rate, and 2-year survival rate were higher in the trial group (paclitaxel and carboplatin combined with bevacizumab) than those in the control group (paclitaxel and carboplatin) (P<0.05); however, the incidences of the adverse reactions, such as leukopenia, hemorrhage, proteinuria and hypertension, etc, were higher in the trial group than those in the control group (P<0.05). There were no statistical differences between the trial group and the control group in the incidences of fatigue, thrombocytopenia, neutropenia or hyponatremia, etc (P>0.05). In addition, the median progression-free survival and overall survival were longer in the trial group than those in the control group.ConclusionFor the treatment of NSCLC, paclitaxel and carboplatin combined with bevacizumab is superior in terms of disease control, overall response and prolonging patient survival, etc, but will be associated with more adverse reactions.

        • CT-guided Hook-wire versus microcoil localization in the pulmonary nodules surgery: A systematic review and meta-analysis

          ObjectiveTo systematically evaluate the application effect of CT-guided Hook-wire localization and CT-guided microcoil localization in pulmonary nodules surgery. MethodsThe literatures on the comparison between CT-guided Hook-wire localization and CT-guided microcoil localization for pulmonary nodules were searched in PubMed, EMbase, The Cochrane Library, Web of Science, Wanfang, VIP and CNKI databases from the inception to October 2021. Review Manager (version 5.4) software was used for meta-analysis. The Newcastle-Ottawa Scale (NOS) was used to evaluate the quality of studies.ResultsA total of 10 retrospective cohort studies were included, with 1 117 patients including 473 patients in the CT-guided Hook-wire localization group and 644 patients in the CT-guided microcoil localization group. The quality of the studies was high with NOS scores>6 points. The result of meta-analysis showed that the difference in the localization operation time (MD=0.14, 95%CI ?3.43 to 3.71, P=0.940) between the two groups was not statistically significant. However, the localization success rate of the Hook-wire group was superior to the microcoil group (OR=0.35, 95%CI 0.17 to 0.72, P=0.005). In addition, in comparison with Hook-wire localization, the microcoil localization could reduce the dislocation rate (OR=4.33, 95%CI 2.07 to 9.08, P<0.001), the incidence of pneumothorax (OR=1.62, 95%CI 1.12 to 2.33, P=0.010) and pulmonary hemorrhage (OR=1.64, 95%CI 1.07 to 2.51, P=0.020). ConclusionAlthough Hook-wire localization is slightly better than microcoil localization in the aspect of the success rate of pulmonary nodule localization, microcoil localization has an obvious advantage compared with Hook-wire localization in terms of controlling the incidence of dislocation, pneumothorax and pulmonary hemorrhage. Therefore, from a comprehensive perspective, this study believes that CT-guided microcoil localization is a preoperative localization method worthy of further promotion.

        • Research progress of angiotensin converting enzyme 2 co-expression in non-small cell lung cancer and SARS-CoV-2

          Since the first case of corona virus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) at the end of 2019, the virus has spread rapidly around the world and has become a global public health problem. In the process of this virus epidemic, compared with the general population, cancer patients are considered to be highly susceptible people, especially the lung cancer patients. Some studies have shown that angiotensin converting enzyme 2 (ACE2) may be the pathway for SARS-CoV-2 to infect the host. At the same time, ACE2 is often abnormally expressed in non-small cell lung cancer. Therefore, understanding the respective mechanisms of ACE2 in COVID-19 and non-small cell lung cancer has extremely important reference value for the study of vaccines and therapeutic drugs, and also provides meaningful guidance for the protection of patients with lung cancer during the epidemic. This article reviews the possible invasive mechanism of ACE2 in SARS-CoV-2 and its abnormal expression in non-small cell lung cancer.
