To prevent and control 2019 novel coronavirus pneumonia diseases (COVID-19), hundreds of medical teams and tens of thousands of medical professionals throughout the nation were transferred to Hubei to assist COVID-19 control efforts. Medical professionals were at high risk of novel coronavirus pneumonia infections. To ensure the prevention and control of infection in medical teams and prevent cross-infection among medical staff at the medical station, this management standard includes routine management standards, resident disinfection, personnel entry and exit process, and logistics support management, so as to provide reference for medical teams combating COVID-19 in the future.
In recent years, along with more importance having been given by health care facilities and health administrative departments nationally, the work force of infection prevention and control is constantly increasing. In the new era, to help infection prevention and control practitioners and all health care workers make the right direction in infection prevention and control professional business and make sure the infection prevention and control measures are implemented, what we need is to define the target of infection prevention and control scientifically, identify obligation subjects, and improve the infection prevention and control system and working mechanism from the top-level.
Healthcare-associated infections are common adverse events in healthcare settings, causing significant morbidity and mortality. There has been a significant increase in the commitment to Infection Prevention and Control worldwide in recent years. This important role and responsibilities are relying on the infection prevention and control practitioners (IPCPs). The competencies of IPCP is thus critical to the success of infection surveillance, prevention, and control program. IPCPs must ensure that they are competent in addressing the challenges they face and are equipped to be competent in fulfilling their duties. Core competencies and professional development of IPCP differ among countries while some are more progressive than the others. This review is aiming to be a reference to develop IPCP core competencies framework and professional development conducive for Asia Pacific region.
Objective To design a nucleic acid information early warning system and evaluate the effect of the application. Methods 60 nurses working in the clinical department of Zhongnan Hospital of Wuhan University from August to October 2021 were selected by convenient sampling method, and the included nurses were divided into experimental group and control group. The experimental group used the nucleic acid information early warning system, and the control group used the nucleic acid registration in a common way. Statistical duration of the nucleic acid information, nucleic acid leakage detection rate and nurses job satisfaction after application were compared between the two groups. Results After the application of nucleic acid information early warning system, compared with the control group, the statistical duration of the nucleic acid information in the experimental group was shortened [(2.0±0.4) vs. (3.8±0.7) min; t=25.827, P<0.001], nucleic acid leakage detection rate decreased [1.4% vs. 6.9%; P=0.019]. The job satisfaction of the experimental group was higher than that of the control group (P<0.001). Conclusions Applying the nucleic acid information early warning system could reduce statistical duration of the nucleic acid information, nucleic acid leakage detection rate and improve nurses job satisfaction and work experience. It will provide reference and basis for medical institutions to normalize the management of nucleic acid prevention and control in medical institutions.
Objective To evaluate the current status of human resources in healthcare-associated infection prevention and control (infection control) in Jiangxi Province, and explore the impact of emergency public health events on the human resources of infection control professionals in various levels and types of medical institutions. Methods From October 1st to 31st, 2023, questionnaire and on-site interviews were conducted to investigate the human resources situation of infection control professionals in various levels and types of medical institutions in Jiangxi Province. Three stages were selected for the investigation: before the outbreak of COVID-19 (before the event, December 2019), during the event (June 2022), and after the transition of COVID-19 (after the event, June 2023), focusing on the characteristics of human resources between before the event and after the event by the comparative analysis. Results Finally, 289 medical institutions were included. There was a statistically significant difference in the number of infection control professionals in medical institutions among 2019, 2022, and 2023 (χ2=189.677, P<0.001). The number of infection control professionals in 2019 was lower than that in 2022 (P<0.001) and 2023 (P<0.001), but there was no statistically significant difference between 2022 and 2023 (P=0.242). The number of infection control professionals per thousand beds in 2019, 2022, and 2023 was 4.40, 6.16, and 5.76, respectively. There was no statistically significant difference between 2019 and 2023 in terms of professional titles, gender, educational level, or professional background (P>0.05). Conclusion Emergency public health events have promoted the increase in the number of infection control professionals, but there is no statistical significance in the professional titles, educational level, or professional background of infection control professionals.
ObjectiveTo analyze the status and influencing factors of awareness of diabetic retinopathy (DR) knowledge in diabetics in Funing County, Jiangsu Province. MethodsThis is a cross-sectional study. In 2021, a total of 2 125 residents with diabetes in Funing County, Jiangsu Province were recruited using cluster random sampling methods. General examinations were performed and ophthalmological evaluation were conducted including visual acuity in daily life, slit lamp microscope with preset lens, digital non-mydriatic fundus photography. Clinical assessment and classification of DR were according to the international standard clinical classifications of diabetic retinopathy in 2019. DR was classified into no obvious DR, mild non-proliferative diabetic retinopathy (NPDR), moderate NPDR, severe NPDR and proliferative diabetic retinopathy (PDR). Field questionnaire surveys were conducted to show the awareness rate of DR knowledge. Then Chi-square test and Logistic regression analysis were used to investigate influencing factors. ResultsAmong 2 125 subjects, 762 (35.86%, 762/2 125) residents were male and 1 363 (64.14%, 1 363/2 125) residents were female. A total of 444 (20.89%, 444/2 125) subjects were diagnosed as DR. Among which, 438 (20.61%, 438/2 125) subjects were NPDR, 6 (0.28%, 6/2 125) subjects were PDR. A total of 419 (19.72%, 419/2 125) subjects were diagnosed as cataract. The topic awareness rates were all below 45.00%. The overall population awareness rate was 19.45% (4 959/25 500). Awareness rate was correlated with age, visual acuity in daily life, education level, family history of diabetes, diabetic retinopathy grades and the type of treatment for diabetes (P<0.001). Logistic regression analysis showed that the knowledge rate of prevention and treatment of DR Decreased significantly in patients aged 60-79 compared with those aged under 50 (P<0.05). Compared with those with visual acuity<0.1 in daily life, the knowledge rate of DR prevention and treatment in those with visual acuity≥0.1 was significantly decreased (P<0.05). While primary school to college education, with family history of diabetes, with DR, oral hypoglycemic agents and/or insulin treatment were associated with higher awareness rate (P<0.05). ConclusionsThe DR knowledge level is low among diabetics in Funing County, Jiangsu Province in 2021. Age, visual acuity in daily life, education level, family history of diabetes, with DR and receiving drug treatments are main influencing factors for diabetic’s awareness.
Objective To understand the influence of organisational support and professional identity of hospital infection prevention and control (HIPC) practitioner in public hospitals. Methods HIPC practitioner of the public medical institutions secondary level and above in Neijiang City, Sichuan Province were used as the research object. A questionnaire survey was used to analyze their organizational support and professional identity in May 2022, to compare the differences in organizational support and professional identity among HIPC practitioner with different positions, working ages, and majors, and analyze the correlation between organizational support and professional identity among HIPC practitioner. Results A total of 90 valid questionnaires were retrieved from investigation. The average scores for organizational support and professional identity among HIPC practitioner were (3.409±0.625) and (2.518±0.504) points. Among them, the organizational support of intermediate professional title personnel [(3.155±0.785) points], personnel with 5-10 years of service [(3.071±0.696) points], and clinical physicians [(3.342±0.860) points] were the lowest; the professional identity of personnel with intermediate professional titles [(2.357±0.477) points], those with more than 10 years of service[(2.431±0.454) points], and other professionals [(2.471±0.260) points] were the lowest. The Pearson correlation analysis results showed that there was a positive correlation between organizational support and professional identity (r=0.490, P<0.05), self-identity and work support (r=0.364, P<0.05), and environmental identity and relationship interests (r=0.698, P<0.05). Conclusions Due to the low level of organizational support, there is a decrease in professional identity and significant job burnout, which in turn leads to an increase in the turnover rate of infection control professionals. Therefore, it is necessary to enhance the sense of organizational support for specialized personnel in infection control to promote their professional identity and stabilize the infection control team. Suggest providing necessary support at the organizational level and self-improvement at the individual level.
Novel coronavirus pneumonia is a new type of respiratory infectious disease that has rapidly spread in many countries or regions around the world. The World Health Organization (WHO) named it “coronavirus disease 2019 (COVID-19)”. Glucocorticoids (GC) have certain application value in patients with COVID-19, but they need to be used with caution and strict indications and dosage. Application of large doses of GC can also cause osteonecrosis of femoral head (ONFH). On the basis of the latest literature and evidence-based medical evidence on the fight against COVID-19 epidemic and steroid ONFH diagnosis and treatment, the Bone Circulation and Osteonecrosis Professional Committee, Shockwave Medical Specialty Committee of Chinese Research Hospital Association organized Chinese bone necrosis related experts to jointly write this consensus, focusing on the prevention strategy and the protective management measures in the ONFH diagnosis and treatment process during the prevention and control of COVID-19, which can provide reference for hospitals at all levels to carry out early prevention and treatment of ONFH.
When a clustered coronavirus disease 2019 epidemic occurs, how to prevent and control hospital infection is a challenge faced by each medical institution. Under the normalization situation, building an effective prevention and control system is the premise and foundation for medical institutions to effectively prevent and control infection when dealing with clustered epidemics. According to the principles of control theory, medical institutions should quickly switch to an emergency state, and effectively deal with the external and internal infection risks brought by clustered epidemics by strengthening source control measures, engineering control measures, management control measures and personal protection measures. This article summarizes the experience of handling clustered outbreaks in medical institutions in the prevention and control of coronavirus disease 2019, and aims to provide a reference for medical institutions to take effective prevention and control measures when dealing with clustered outbreaks.
ObjectiveTo explore the distribution of multidrug resistant organism in neonates admitted to the hospital through various ways, and analyze the risk factors in order to avoid cross infection of multidrug resistant organism in neonatology department.
MethodsA total of 2 124 neonates were monitored from January 2012 to July 2013, among which 1 119 were admitted from outpatient department (outpatient group), 782 were transferred from other departments (other department group), and 223 were from other hospitals (other hospital group). We analyzed their hospital stays, weight, average length of stay, and drug-resistant strains, and their relationship with nosocomial infection.
ResultsAmong the 105 drug-resistant strains, there were 57 from the outpatient group, 27 from the other department group, and 21 from the other hospital group. The positive rate in the patients transferred from other hospitals was the highest (9.42%). Neonates with the hospital stay of more than 14 days and weighing 1 500 g or less were the high-risk groups of drug-resistant strains in nosocomial infection. Drug-resistant strains of nosocomial infection detected in the patients admitted through different ways were basically identical.
ConclusionWe should strengthen screening, isolation, prevention and control work in the outpatient neonate. At the same time, we can't ignore the prevention and control of the infection in neonates from other departments or hospitals, especially the prevention and control work in neonates with the hospital stay of more than 14 days and weighing 1 500 g or less to reduce the occurrence of multiple drug-resistant strains cross infection.
