        • Interventional embolization, surgery and high intensity focused ultrasound for the treatment of uterine fibroids: a network meta-analysis

          ObjectivesTo systematically review the safety and effectiveness of uterine artery embolization (UAE), surgery and high intensity focused ultrasound (HIFU) in the treatment of uterine fibroids.MethodsPubMed, EMbase, The Cochrane Library, Web of Science, WanFang Data and CNKI databases were electronically searched to collect relevant studies on comparing the safety and effectiveness of UAE, surgery and HIFU in the treatment of uterine fibroids from January 2000 to August 2019. Two reviewers independently screened the literature, extracted the data and evaluated the risk of bias of included studies, network meta-analysis was performed by ADDIS 1.16.8 software and Stata 14.0 software.ResultsA total of 11 trials (22 references) involving 3469 patients were included. Compared with surgery, UAE and HIFU patients had higher quality of life (1-year follow-up) improvement, and UAE was higher than HIFU. Network meta-analysis showed that patients treated with HIFU had the lowest incidence of major complications within 1 year, followed by UAE, and surgery. Patients treated with HIFU and UAE had shorter hospitalization and quicker recovery time than surgery. The rate of further intervention after surgery treatment might be lower than that of UAE and HIFU.ConclusionsUAE has the highest quality of life improvement (1-year follow-up) for uterine fibroids. HIFU and UAE are safer with shorter hospital stays and quicker recovery time compared with surgery. However, both UAE and HIFU have the risk of re-treatment. However, limited by the number and quality of included studies, the above conclusions are needed to be verified by more high-quality studies.

        • Application of Pencil Beam Model Based on Point Kernel in Intensity Modified Radiatioin Therapy

          In this paper, a method for dose calculation with pencil beam kernels constructed by point kernel superposition was proposed to accelerate the dose calculation during intensity optimization iteration. With this method, the direct aperture optimization method can be integrated in the planning system based on point kernel convolution/superposition model. The dose calculation time was also reduced during the iteration. From the result of the phantom and clinical patient data test, it was concluded that this method could be used for the intensity optimization of iteration dose calculation as the satisfied precision due to the optimization result coherence obtained. By implementing the method in the planning system product based on point kernel convolution/superposition model, a lot of additional research and development works for the pencil beam dose calculation model as well as the product maintenance cost can be avoided.

        • Influence of resistance training intensity on glycosylated hemoglobin in elderly patients with type 2 diabetes: a meta-analysis

          Objectives To systematically analyze the effect of high-intensity and low-intensity resistance training on glycosylated hemoglobin (HbA1c) in elderly patients with type 2 diabetes. Methods PubMed, EBSCO, Cochrane Library, Web of science, Wanfang, Chinese National Knowledge Infrastructure and Chongqing VIP were searched to collect randomized controlled trials of resistance training intervention in elderly patients with type 2 diabetes. The search time limit was from the establishment of the database to August 28, 2021, and the RevMan 5.3 and Stata 15.0 software were used for meta-analysis. Results A total of 415 patients in 8 articles were included in this study. A subgroup analysis based on control measures showed that compared with the resistance training group, the HbA1c level effect value of the non- training group was weighted mean difference (WMD) = ?0.54% [95% confidence interval (CI) (?1.06%, ?0.03%), P=0.04)], the HbA1c level effect value of the flexibility training group was WMD=?0.47% [95%CI (?1.05%, 0.12%), P=0.12], the HbA1c level effect value of the aerobic training group was WMD=?0.10% [95%CI (?0.71%, 0.51%), P=0.75]. Subgroup analysis of resistance strength showed that compared with the flexibility or non-resistance training group, both high-intensity [WMD=?0.99%, 95%CI (?1.16%, ?0.81%), P<0.00001] and low- and medium-intensity resistance training [WMD=?0.29%, 95%CI (?0.58%, ?0.01%), P=0.05] can reduce HbA1c. Compared with the high-intensity resistance training, the HbA1c level effect value of the low- and medium-intensity resistance training was WMD=0.03% [95%CI (?0.33%, 0.39%), P=0.88]. Compared with the low- and medium-intensity resistance training, the HbA1c level effect value of the low- and medium-intensity aerobic training was WMD=?0.10% [95%CI (?0.71%, 0.51%), P=0.75]. Among all the included studies, only the high-intensity resistance training group reported multiple adverse events.ConclusionsCurrent evidence suggests that elderly patients with type 2 diabetes are more suitable to choose low-to-medium-intensity resistance training to control HbA1c. It is recommended that elderly patients with type 2 diabetes take 60% to 75% of the maximum muscle strength of medium-intensity resistance training.

        • Analysis of Clinical Effects about High Intensity Focused Ultrasound in Treatment of 8 Hepatic Echinococosis

          ObjectiveTo explore the safety and effectiveness of high intensity focused ultrasound (HIFU) in treatment of hepatic echinococcosis. MethodsThe clinical data of 8 patients with hepatic echinococcosis from 2008 to 2010 in the HIFU treatment center of The Second Affiliated Hospital of Chongqing Medical University were analyzed retrospectively, the effect of HIFU treatment, postoperative liver area pain, fever, skin burn and other complications were evaluated, the prognosis and recurrence were followed-up. ResultsAll the clinical manifestations and imaging manifes-tations of these 8 patients were improved after HIFU treatment. The liver functions of all the patients were injuried, which all were returned to normal within 5 days. Three patients suffered from fever and recovered within 6 days. Six patients appeared pain of liver area and relieved within 7 days. Operation area skin was basically normal, except 3 cases of mild swelling, no special complications happened. ConclusionFrom the limited data initially shows that it is a safe and effec-tive method of HIFU in treatment of hepatic echinococcosis, which is little invasion and less postoperative complications.

        • A meta-analysis of the comparison of intervention effects of high-intensity interval training and moderate-intensity continuous training on patients with metabolic syndrome

          Objective To explore the difference of intervention effect between high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) on patients with metabolic syndrome (MetS). Methods China National Knowledge Infrastructure, WanFang Data, PubMed, Web of Science and EBSCO were searched for randomized controlled trials (RCTs) till May 2022. Two reviewers independently reviewed the literature, extracted data, and assessed the risk of bias of included RCTs. Comprehensive Meta-Analysis software was used for meta-analysis. Result A total of 5 RCTs were included, including 216 patients. The results of meta-analysis showed that: except fasting blood glucose, high-density lipoprotein cholesterol, systolic blood pressure, waist circumference, body mass index and body fat percentage (P>0.005), low-density lipoprotein cholesterol [mean difference (MD)=?7.487 mg/dL, 95% confidence interval (CI) (?12.543, ?2.431) mg/dL, P=0.004], total cholesterol [MD=?11.487 mg/dL, 95%CI (?16.523, ?6.452) mg/dL, P<0.001], triglycerides [MD=?26.296 mg/dL, 95%CI (?50.557, ?2.035) mg/dL, P=0.034] and diastolic blood pressure [MD=?2.770 mm Hg (1 mm Hg=0.133 kPa), 95%CI (?5.131, ?0.409) mm Hg, P=0.021] of HIIT were better than MICT. Conclusion In terms of blood glucose indicators and morphological indicators, the effect of HIIT group and MICT group was similar, but the effect of HIIT on blood lipid indicators and blood pressure indicators of patients with MetS was better than MICT.

        • Differential diagnosis significance of 3.0T MRI united-sequence examination forbenign and malignant breast lesions

          Objective To explore the differential diagnosis significance of 3.0T MRI united-sequences examination in the diagnosis of benign and malignant breast lesions. Methods A total of 67 breast lesions of 59 patients were collected prospectively, which be treated at the Sichuan Provincial People’s Hospital during July 2015 to January 2017. All patients were underwent bilateral breast 3.0T magnetic resonance plain scan, diffusion weighted imaging, and dynamic enhanced scan successively before surgical operation. Analysis of morphological features of the benign and malignant breast lesions, the time-signal intensity curve (TIC), the apparent diffusion coefficient (ADC), and the combination diagnosis of them were performed. Results Of all 59 patients, 67 lesions were confirmed by histopathology, including 18 benign lesions and 49 malignant lesions. The morphological features (including margin, shape, border, and evenness), the types of TIC of dynamic enhancement, and ADC value between the benign lesions and malignant lesions were statistically significant (P<0.05). The sensitivity and specificity of Fischer scoring system was 89.8% (44/49) and 61.1% (11/18) respectively. The sensitivity and specificity of TIC types was 83.7% (41/49) and 77.8% (14/18) respectively. The diagnostic threshold of ADC value was 1.012×10–3 mm2/s, with the sensitivity and specificity for the diagnosis was 91.8% (45/49) and 83.3% (15/18) respectively. The sensitivity and specificity of the combination of Fischer scoring system and TIC type for diagnosis between benign and malignant breast lesions was 95.9% (47/49) and 72.2% (13/18) respectively. The sensitivity and specificity of the combination of Fischer scoring system, TIC type, and ADC value for benign and malignant breast lesions was 98.0% (48/49) and 83.3% (15/18) respectively. Conclusion The combination of Fischer scoring system, TIC type, and diffusion-weighted imaging for the differential diagnosis between benign lesions and malignant lesions was more effective than single imaging method.

        • Therapy-related Complication Incidences of Warfarin with Different Anticoagulation Intensity in Chinese after Mechanical Heart Valve Replacement: A Systematic Review

          ObjectiveTo systematically review therapy-related complications incidences of warfarin with different anticoagulation intensity in Chinese after mechanical heart valve replacement, and to explore the suitable anticoagulation intensity of warfarin. MethodsWe electronically searched The Cochrane Library (Issue 2, 2013), EMbase, PubMed, CNKI, CBM, WanFang Data and VIP for studies about therapy-related complications rates of warfarin with different anticoagulation intensity in Chinese after mechanical heart valve replacement from inception to March 2013. Two reviewers independently screened literature according to inclusion and exclusion criteria, extracted data, and assessed methodological quality of included studies. Then meta-analysis was performed using RevMan 5.2. ResultsA total of 7 studies involving 3 787 cases were finally included, 2 985 cases in Group A (targeted INR≤2.3) and 802 cases in Group B (targeted INR > 2.3). The results of meta-analysis showed that, compared with Group B, the total therapy-related complications incidence of warfarin with different anticoagulation intensity significantly decreased in Group A (OR=0.35, 95%CI 0.28 to 0.44, P < 0.000 01). The incidence of hemorrhage in Group A (OR=0.15, 95%CI 0.08 to 0.27, P < 0.000 01) was lower than that in Group B. However, the incidence of embolism in Group A was higher than that in Group B (OR=2.77, 95%CI 1.54 to 4.98, P=0.000 7). ConclusionApplying low-intensity anticoagulation for Chinese patients after mechanical heart valve replacement could reduce the overall incidences of anticoagulation-related complications and hemorrhage, but it could also significantly increase the incidence of thrombosis.

        • Study on the distribution characteristics of bone metabolic markers in plateau builders and the influencing factors

          ObjectiveTo study the distribution of bone-specific alkaline phosphatase (BALP), type Ⅰ collagen cross-linked C-telopeptide (CTX) and tartrate-resistant acid phosphatase (TRAP)-5b in plateau area builders, and analyze the influencing factors under plateau environment. MethodsBetween April and May, 2014, using random stratified cluster sampling, we included in our study 650 blood samples from the power grid construction people in Batang County of Ganzi Autonomous Prefecture of Sichuan Province and Mangkang County of Tibet Autonomous Region with an altitude ranging from 2 600 to 4 450 meters, averaging (3 586.50±610.85) meters. We collected their fasting blood and detected their TRAP-5b, CTX and BALP by enzyme-linked immunosorbent assay method. By using SPSS 13.0, we analyzed the relationship between TRAP-5b, CTX, BALP and the influencing factors such as age, working intensity, residence time in the plateau area and altitude of the plateau. In the end, we tried to find out the main influencing factors of bone metabolic markers in the plateau environment.ResultsThe levels of CTX, BALP and TRAP-5b were the highest before the age of 20, and the average levels of them were respectively (1.04±0.38) ng/mL, (52.09±14.62) μg/L, and (4.22±1.38) U/L. With the increase of age, the levels of CTX, TRAP-5b and BALP showed a downward trend, but CTX and BALP reached the lowest level in the age group of 40 to 49 years old, and the average levels of CTX and BALP were (0.44±0.26) ng/mL and (24.77±9.89) μg/L, respectively. Then they gradually increased after the age of 50. TRAP-5b reached the lowest level in the age group of 30 to 39 years old, and the average level of TRAP-5b was (2.59±0.95) U/L. Then it gradually increased after the age of 40. The activity of CTX and BALP increased obviously with the increase of altitude. With the increase of labor intensity, BALP, TRAP-5b and CTX all increased. However, no matter what labor intensity, the bone formation marker BALP first increased and then decreased with the plateau residence time, while the bone absorption marker TRAP-5b increased after the first reduction.ConclusionsBone metabolic markers are different in different age groups. Altitude, working intensity and plateau working time have significant effects on bone metabolism markers.

        • Drinkers engaged in moderate to high-intensity work are less tolerant to bronchoscopy under mild to moderate sedation

          ObjectiveTo investigate the effects of alcohol consumption status and labor intensity on the tolerance of patients undergoing bronchoscopy under mild to moderate sedation anesthesia. MethodsAdult patients scheduled for bronchoscopy under local anesthesia or mild to moderate sedation were included, and their clinical characteristics and bronchoscopy tolerance were recorded. The study compared changes in vital signs, severity of coughing, and procedure failure rates during bronchoscopy among patients with different drinking statuses (non-drinkers, former drinkers, and current drinkers) and different labor intensities (light, moderate, and heavy labor intensity) to assess the impact of drinking status and labor intensity on bronchoscopy tolerance.ResultsA total of 142 subjects were included in the study, with 50 patients receiving local anesthesia and 92 patients receiving mild to moderate sedation. Current drinkers had a higher procedure failure rate (2.9% vs. 1.9% vs. 25.0%, P<0.01, for non-drinkers, former drinkers, and current drinkers, respectively) and more significant intraoperative blood pressure drops (systolic pressure change, ?3.5±9.1 vs. ?0.2±8.1 vs. ?9.3±17.9 mm Hg, P<0.01; diastolic pressure change, ?0.5±5.6 vs. 2.9±7.9 vs. ?3.2±12.4, P<0.05). Patients with moderate to heavy labor intensity also had higher procedure failure rates and more pronounced intraoperative blood pressure drops (procedure failure rate, 1.1% vs. 13.2% vs. 22.2%, P<0.01, for light, moderate, and heavy labor intensity, respectively; intraoperative systolic pressure change, ?1.8±8.5 vs. 2.8±8.5 vs. ?17.2±24.7 mm Hg, P<0.001; intraoperative diastolic pressure change, 1.3±6.4 vs. 0.2±6.7 vs. ?8.1±17.2 mm Hg, P<0.01). The impact of drinking status and labor intensity on procedure tolerance was only observed in patients receiving mild to moderate sedation. After controlling for relevant confounding factors, current drinking and moderate to heavy labor intensity were identified as independent risk factors for procedure failure (current drinkers compared to non-drinkers or former drinkers, OR 47.2, 95%CI: 3.1-232.2, P<0.05; moderate to heavy labor intensity compared to low labor intensity, OR 25.7, 95%CI: 2.8-67.7, P<0.05).ConclusionsCurrent drinkers engaged in moderate to heavy labor intensity are less likely to tolerate bronchoscopy under mild to moderate sedation. It is essential to pay attention to the selection and evaluation of anesthesia methods, procedure types, and intraoperative monitoring for this population.

        • High-intensity intermittent aerobic exercise on exercise capacity and quality of life in patients with heart failure: a meta-analysis

          ObjectivesTo systematically review the efficacy of high-intensity intermittent exercise (HIIE) on cardiac function, exercise capacity, quality of life and depression in patients with heart failure.MethodsPubMed, Web of Science, The Cochrane Library, EBSCOhost, EMbase, CBM, CNKI, WanFang Data and VIP databases were electronically searched to collect randomized controlled trials (RCTs) on HIIE on cardiac function, exercise capacity, quality of life and depression in patients with heart failure from inception to April, 2019. Two reviewers independently screened literature, extracted data, and assessed risk of bias of included studies. Then, RevMan 5.3 software and Stata 15.1 software were used for meta-analysis.ResultsA total of 16 RCTs involving 549 patients were included. The results of meta-analysis showed that, compared with the control group, HIIE could increase peak oxygen consumption (MD=2.04, 95%CI 0.74 to 3.33, P=0.002), peak work rate (MD=12.85, 95%CI 1.17 to 24.52, P=0.03), left ventricular ejection fraction (MD=4.24, 95% CI 1.40 to 7.07, P=0.003), quality of life (MD=7.32, 95%CI 1.41 to 13.22, P=0.02), and the six minute walk distance (MD=42.46, 95%CI 20.40 to 64.52, P=0.000 2). However, there was no significant difference between two groups in the depression score (SMD=0.39, 95%CI ?0.52 to 1.31, P=0.40) and VE/VCO2 Slope (MD=0.12, 95%CI ?1.02 to 1.26, P=0.84).ConclusionsCurrent evidence shows that compared with routine exercise or moderate intensity exercise, HIIE can improve exercise capacity, quality of life and cardiac function in patients with heart failure, but there is no significant difference in improving depression. Due to limited quality and quantity of the included studies, more high quality studies are required to verify above conclusion.
