        • The Changes and Measures Against the Glucose in the Operation of the Diabetes by TURP

          摘要:目的:探討經尿道前列腺電切術中糖尿病患者血糖變化以及處理對策。方法:2006年7月~ 2009年1月共對80例患有前列腺增生合并糖尿病患者行TURP,同期對80例單純性前列腺增生患者進行相同手術,回顧分析其術前、術中30 min、60 min、90 min 指尖血糖變化及干預情況。結果:治療組80例患者,51例術中血糖值明顯低于術前,分別為1.8~3 mmol/L;對照組術前與術中血糖值基本一致,血糖波動于4.5~5.6 mmol/L。結論:糖尿病患者糖的儲備能力差,在行經尿道電切術中易發生低血糖綜合征,術中及時的血糖監測及干預對保證患者的安全有重要意義。Abstract: Objective: To study the changes and measures against the glucose in the operation of the Diabetes by TURP. Methods:Eighty patients with prostate combining diabetes operated from July 2006 to Jan. 2009 were reviewed, and 80 prostate treated at the same period with the same operation measure were selected as control. The preoperative glucose, intraoperative glucose (30′, 60′,90′) of fingertip, and countermeasures were studied and compared between the two groups. Results:Fiftyone cases of the experimental group of intraoperative blood glucose was significantly lower than preoperative values, respectively 1.83 mmol/ L; control group preoperative and intraoperative blood glucose values were basically the same, blood glucose fluctuations in the 4.55.6 mmol/L. Conclusion: The capacity in patients with diabetes is poor, easy to hypoglycemia syndrome in the act of TURP surgery, intraoperative blood glucose monitoring and timely intervention to ensure patient safety significance.

        • Acinetobacter Baumannii Infection in Neurosurgery Intensive Care Unit and the Nursing Countermeasures

          ObjectiveTo explore the infection condition of Acinetobacter baumannii at the Neurosurgery Intensive Care Unit (NICU), and analyze the possible risk factors. MethodsWe retrospectively analyzed the clinical data of Acinetobacter baumannii infection patients with craniocerebral injury treated at the NICU between January 2011 and June 2013. We collected such information as infection patients' population distribution, infection site, invasive operations and patients' nurse-in-charge level and so on, and analyzed the possible risk factors for the infection. ResultsThirty-one patients were infected with Acinetobacter baumannii, and they were mainly distributed between 60 and 80 years old. The main infection site was lower respiratory tract, followed in order by urinary tract, gastrointestinal tract, skin and soft tissue. The risk factors might be related to age, invasive operation, nurse working ability, etc. ConclusionThe patients at the NICU are vulnerable to infection of Acinetobacter baumannii. Reducing invasive diagnosis and nursing procedures, providing optimal care, and carrying out specialized nurse standardization training may be the important means to effectively reduce the infection.

        • Improvement strategies for elderly-friendly medical services under the context of high-quality development: a case study of elderly assistance practices at West China Hospital of Sichuan University

          At present, China has entered an aging society facing the dual challenges of “growing old before becoming affluent” and “aging without adequate preparation”, leading to significant eldercare pressures. With the rapid development of smart healthcare, the elderly population’s diverse, differentiated, and personalized medical demands have become increasingly prominent, presenting new challenges for geriatric healthcare services. This article elaborates and analyzes the elderly healthcare policies and the healthcare challenges faced by the elderly under China’s national strategy for actively addressing population aging. It also introduces the practical experience of intelligent elderly medical assistance in West China Hospital of Sichuan University from the aspects of enhanced appointment coordination mechanisms, refined treatment workflow optimization, and upgraded patient-centered service provisions. The research ultimately proposes actionable management strategies to achieve intelligent continuity in geriatric care delivery.

        • Analysis of 598 Cases of Nosocomial Infection in a New Comprehensive Hospital

          ObjectiveTo understand the characteristics of and risk factors for nosocomial infection in a newly built branch of a university teaching hospital, in order to investigate the control measures for prevention and control of nosocomial infection. MethodsA total of 598 cases of nosocomial infection from April 2012 to June 2014 were enrolled in this study. We analyzed statistically such indexes as nosocomial infection rate, infection site, pathogen detection, and use of antibiotics. Meantime, infection point-prevalence survey was introduced by means of medical record checking and bedside visiting. ResultsAmong all the 44 085 discharged patients between April 2012 and June 2014, there were 598 cases of nosocomial infection with an infection rate of 1.36%. Departments with a high nosocomial infection rate included Intensive Care Unit (ICU) (9.79%), Department of Orthopedics (2.98%), Department of Geriatrics (2.62%), and Department of Hematology (1.64%). The top four nosocomial infection sites were lower respiratory tract (45.32%), urinary tract (13.21%), operative incision (8.86%), and blood stream (8.86%). The samples of 570 nosocomial infections were delivered for examination with a sample-delivering rate of 95.32%. The most common pathogens were acinetobacter Baumanii (17.02%), Klebsiella pneumoniae (14.21%), Escherichia coli (13.68%), Pseudomonas aeruginosa (11.93%), and Staphylococcus aureus (9.12%). And urinary tract intubation (42.81%), admission of ICU (28.60%), and application of corticosteroid and immunosuppressive agents (26.42%) were the top three independent risk factors for nosocomial infection. ConclusionGeneral and comprehensive monitoring is an effective method for the hospital to detect high-risk departments, factors and patients for nosocomial infection, providing a theoretical basis for prevention and control of nosocomial infection.

        • Analysis on prescription dispensing error and its avoidance in outpatient pharmacy

          Objective To analyze outpatient pharmacy internal prescription dispensing errors list and raise suggestions on preventive measures, in order to provide better and safer medical service for patients. Methods We summarized and analyzed the prescription dispensing error types and causes based on 320 cases of internal prescription dispensing errors of the outpatient pharmacy in a hospital of the highest rank between January and June 2014. Then, we put forward suggestions on improvement measures. Six months after the implementation of these measures, we compared the error rate after dispensing between January and June 2014 with those between July and December 2014. Results Among all the 320 prescription dispensing errors, 120 (37.50%) were wrong medication amount, 101 (31.57%) were wrong drugs, 76 (23.75%) were wrong usage and dosage, 17 (5.31%) were wrong packaging specification, and 6 (1.87%) were wrong medication form. The dispensing error rate between July and December 2014 was reduced compared with the rate between January and June 2014. The error rate after dispensing declined from 0.01‰ to 0.006‰. Conclusion Encouraging drug dispensing personnel to issue internal dispensing error recording list for the staff who had errors in dispensing, promoting pharmacists’ professional quality, strengthening the management of outpatient pharmacy, reasonable storage of medicines, enhancing intervention of irrational prescriptions, improving the spatial layout of the pharmacy, and perfecting dispensing error management system, can in a large extent reduce medication errors.

        • Bundle infection prevention and control measures: lessons learnt from Wuhan Leishenshan Hospital

          Wuhan Leishenshan Hospital was built within 12 days during the key period of fighting against coronavirus disease 2019 (COVID-19) in Wuhan. It was a field infectious disease hospital with 1500 beds. Due to the emergency of the epidemic situation, the operation mode of “parallel of construction, acceptance, training and treatment” was employed. During the peak period, nearly 3000 medical workers and 13000 builders worked on the same site. In 67 days, 2 011 patients with COVID-19 were treated. Through the bundle infection prevention and control (IPC) measures, Wuhan Leishenshan Hospital achieved zero infection, zero accident, and low level pollution of SARS-CoV-2 (0.3%) by environment monitoring. The bundle IPC measures of Leishenshan Hospital not only provided prevention and control experience for other field infectious disease hospitals at domestic and abroad during the period of COVID-19, but also put forward ideas and work flow for other medical institutions to deal with emerging infectious diseases.

        • Analysis of 14 Cases of Maternal Mortality and Intervention Measures

          ObjectiveTo explore the corresponding intervention measures to reduce maternal mortality rate by analyzing the causes and problems of maternal deaths. MethodsA retrospective analysis was conducted to analyze all cases of maternal mortality from January 2005 to June 2013 in West China Second University Hospital. ResultsAmong the 14 cases of maternal deaths, the main diseases of the patients were pregnancy complicated with heart disease, hypertensive disorders, obstetric hemorrhage, amniotic fluid embolism and ectopic pregnancy. Four cases got prescriptive prenatal care during pregnancy, accounting for 28.6% (4/14), while 10 cases did not, accounting for 71.4% (10/14). Six patients died in prenatal period which accounted for 42.9% (6/14), while 8 died in postnatal period which accounted for 57.1% (6/14) and 5 died within 24 hours which accounted for 62.5% (5/8). Seven underwent cesarean section and 6 fetuses survived. Two went through trial of labor and no fetus survived. There was no ordered postmortem. ConclusionIntensifying education of prenatal care during pregnancy, improving quality of obstetrical service and diathesis of healthcare professionals, strengthening the supervision of high-risk pregnancy and timely choosing the time and manner of delivery are the main measures to decrease the maternal mortality.

        • Risk Factors and Protective Measures of the Occupation Exposure of Probationer Nurses in Operation Room

          ObjectiveTo explore the risk factors and protective measures of the occupation exposure of probationer nurses in operation room. MethodsThe self-designed questionnaire was used to reveal the risk factors and protective measures of occupation exposure in probationer nurses worked at the operating room during January 2011 and December 2013. ResultsThe female probationer nurses in operating room accounted for 97.7%; the probationer nurses from the secondary specialized school accounted for 53.1%. All the nurses didn't received any occupational protection knowledge training in schools. The practical nurses who failed to follow the regulate operation accounted for 76.8%. The occupational exposure occurred in 457 nurses during the period, which accounted for 44.7% of all the probationer nurses surveyed; the biological occupational exposure accounted for 38.5%. ConclusionTo strengthen the occupation safety education of operation room nurses, strictly regulate the operation procedures, establish the consummation protection measures and regulations, and strengthen their self-protection awareness can effectively reduce the occupation exposure injury in the probationer nurses in operation room.

        • Typical practice model, difficulties and countermeasures of integrated elderly care and medical services in China

          With the aggravation of population aging, the medical and care needs of the elderly have increased significantly. Nowadays the integrated elderly care and medical services have been greatly promoted and various services modes have been developed in different regions. In order to promote the healthy development of integrated elderly care and medical services in China, we summarized the practical experience of integrated elderly care and medical services implemented in various parts of China, including six typical service modes such as elderly care in medical services, medical care in elderly care, cooperation between medical services and elderly care, community radiation, family doctor embedded and “Internet+”. Moreover, we compared their providers, service methods, construction specifications, service standards, regulatory agencies and payment methods. Finally, we analyzed the problems and their causes, and proposed corresponding improvements for outstanding problems in the practice of integrated elderly care and medical services in China.
