        • REPAIR OF LARGE ABDOMINAL WALL DEFECT WITH PEDICLE GRAFT OF GREATER OMENTUM AND POLYPROPYLENE MESH

          Objective To evaluate the outcome of pedicle graft of greater omentum and polypropylene mesh in reconstruction of large defect of abdominal wall caused by surgical incision. Methods From 1994 to 2004, 12 cases of large abdominal wall defects were repaired with pedicle graft of greater omentum and polypropylene mesh after removal of abdominal wall tumor; the defect sizes of abdominal wall ranged from 10 cm×7 cm to 25 cm×17 cm. Results The abdominal wall wound in 12 cases were healed by first intention. After a follow-up of 1 to 5 years, no complications of abdominal hernia, infection and intestine obstruction occurred in all patients. Conclusion It is reliable to repair abdominal wall defect caused by surgical incision with pedicle graft of greater omentum and polypropylene mesh instead of peritoneum.

        • Great Omentum Wrapped Engineer Heart Tissue Transplantation Preserves Cardiac Function in Myocardial Infarction Rats

          Objective Engineer heart tissue (EHT) was constructed with mesenchymal stem cells (MSCs) and poly lacticacidCOglycolic acid (PLGA), and grafted onto the surface of myocardial infarction rats. We hypothesized that great omentum wrapping would increase EHT blood supply and ameliorate EHT microenvironment which is in favor of cardiac collagen remodling and heart function. We hope that omentun wrapped EHT could provide a valuable strategy for surgically myocardial infarction therapy. Methods MSCs were isolated from SD rats.Eight weeks after SD rats were subjected to left anterior descending (LAD) ligation, 18 rats were enrolled and divided into three groups, group A(n=6): great omentum wrapped MSCsPLGA EHT implantation; group B (n=6):MSCsPLGA EHT implantation; control group (n=6): the myocardial infarction; the sham group (n=6): only opened and closed chest, underwent LAD ligation, but no EHT implantation. Four weeks after transplantation, the following variables were evaluated: specimen stained with picrosirius red, left ventricle function evaluated by echocardiography, infarction ventricular wall motion by color kinesis (CK). Results Hearts of group A showed significantly less fibrosis than group B and control group (Plt;0.05). Infarction ventricular wall motion assessed by CK indicated significantly improvement in group A compared with group B and control group (Plt;0.05). Four weeks after transplantation, cardiac echocardiography showed left ventricle ejection fraction was lower in control group and group B compared with group A (Plt;0.05). Conclusion Transplantation of MSCsPLGA EHT with great omentum wrapping ameliorated infarction ventricular collagen remodeling, ameliorated infarction ventricular wall motion and preserved left ventricular function.

        • REPAIR OF SPONTANEOUSRUPTURE OF ESOPHAGUSBY PEDICL ED GREATER OMENTUM(10 Cases Re- port)

          OBJECTIVE To evaluate the effect of various covering tissues for improving the cure rate of spontaneous rupture of esophagus. METHODS From 1970 to 1994, 13 cases with spontaneous rupture of esophagus were performed primary repair, among them, 10 cases were covered by pedicled greater omentum after impair, and the other 3 cases were covered by pedicled pleural flap. RESULTS: Satisfactory result and complete recovery were obtained in all 10 cases by using pedicled greater omentum. Two cases among 3 cases using pedicled pleural flap suffered re-rupture of esophagus at 5 days and 8 days after operation, and died because of whole body exhaustion. CONCLUSION Pedicled greater omentum is a good covering tissue for repair of spontaneous rupture of esophagus.

        • Application of pedicled omentum flap in breast reconstruction of breast cancer patients

          Objective To explore the clinical application of the pedicled omentum flap in breast reconstruction of breast cancer patients. Methods Between May 2013 and October 2017, 205 patients with breast cancer received modified mastectomy. The pedicled omentum flap was used to reconstruct breast at the same time. All patients were female with an average age of 34.9 years (mean, 26-58 years). The tumor located at left breast in 127 cases and right side in 78 cases. The diameter of the tumor was 2-5 cm (mean, 2.9 cm). The 120 cases of breast cancer were at stage Ⅰ and 85 cases were at stage Ⅱ; and 126 cases were invasive ductal carcinoma and 79 cases were invasive lobular carcinoma. The course of disease ranged from 10 to 92 days (mean, 38.5 days). The size of defect after tumor ablation ranged from 9 cm× 6 cm to 18 cm×12 cm; the size of pedicled omentum flap ranged from 18 cm×10 cm to 22 cm×16 cm. Results According to the anatomical basis, the omentum was divided into 4 types, including thin type (42 cases, 20.5%), medium type (133 cases, 64.9%), hypertrophy type (24 cases, 11.7%), and absence type (6 cases, 2.9%). All omentum flaps survived successfully and the incisions healed by first intention. All patients were followed up 6-74 months (mean, 24.5 months); 83 cases were followed up more than 5 years. The shape, texture, and elasticity of the reconstructed breast were good and no flap contracture deformation happened. Only linear scar left at the donor sites, and the function of abdomen was not affected. No local recurrence happened. Conclusion The pedicled omentum flap can be harvested safely and reliable, which is the one of ideal option for breast reconstruction in breast cancer patients.

        • Advances in intra-abdominal vascularized lymph node transfer for treatment of limb lymphedema

          Objective To summarize recent clinical advances in intra-abdominal vascularized lymph node transfer (VLNT) for treatment of limb lymphedema. Methods A comprehensive review of the literature on intra-abdominal VLNT was conducted, outlining its historical development, relevant anatomy, and clinical applications, with emphasis on clinical outcomes. Results Intra-abdominal donor sites, such as the omentum and mesentery, are rich in lymphoid tissue and exhibit favorable anti-infective properties. Intra-abdominal VLNT has been widely applied in clinical practice internationally, while reports from China remain limited. Clinical studies indicate that intra-abdominal VLNT can effectively reduce limb swelling, lower infection rates, and improve quality of life in patients with limb lymphedema. It can also be combined with lymphaticovenular anastomosis, reductive procedures, and breast reconstruction to further alleviate symptoms and enhance patient-reported outcomes. Conclusion Intra-abdominal VLNT holds clinical application potential for the treatment of limb lymphedema, but more rigorous, standardized clinical studies are needed to further verify its feasibility and efficacy.

        • TRANSFER OF VASCULARIZED GREATER OMENTUM IN REPAIRING CHRONIC OSTEOMYELITIS WITH SKIN DEFECT

          Greater omentum was chosen to treat chronic osteomyelitis in 12 cases. The involved bones included cranium in 5, tibia and fibula in 4 and metatarsus in 3. After thorough debridement, all patients had partial defects of bone, skin and soft tissue. The defect was filled with vascularized autogenous greater omentum by anastomosing with the host vessels. Theskin defect was covered with medium-thickness skin graft. After 3~9 years follw-up, no recurrence of osteomyelitis and no abdominal symptoms were observed in 11 cases. The osteomyelitis recurred in 1 case at 4 months after operation and died of squamous carcinoma 1 year later. It was suggested that transfer of vascularizedautogenous greater omentum was valuable in treating chronic osteomyelitis.
