Children with retinoblastoma (RB) typically survive their cancer due to advances in early diagnosis and treatment. Extraocular invasion and metastasis, and secondary malignant tumor carry a very high mortality rate. Prerequisites for metastasis include tumor initiating capacity, altered cellular adhesion and cell motility, resistance to extracellular death signals and disruption of the basement membrane and extracellular matrix. All those changes can be determined by the cell of origin and the genetic instability of the tumor, responding to the multiple layers of pressure such as hypoxia, from the tumor microenvironment or niche. The interaction between tumor cells and the tumor stroma is regulated by several metastasissuppressor proteins and microRNA. This knowledge has important implications for our understanding and the treatment of extraocular spreading of RB.
To prevent and control 2019 novel coronavirus pneumonia diseases (COVID-19), hundreds of medical teams and tens of thousands of medical professionals throughout the nation were transferred to Hubei to assist COVID-19 control efforts. Medical professionals were at high risk of novel coronavirus pneumonia infections. To ensure the prevention and control of infection in medical teams and prevent cross-infection among medical staff at the medical station, this management standard includes routine management standards, resident disinfection, personnel entry and exit process, and logistics support management, so as to provide reference for medical teams combating COVID-19 in the future.
Dense breast is an independent risk factor for females with breast cancer, and also one of the important reasons for decreased sensitivity of mammography and increased risk of missed diagnosis. Females with dense breast in China account for a high proportion, making the breast cancer screening a key and difficult point in the prevention and control targeting breast cancer in such population. The formulation of specialized screening expert consensus for dense breast populations is conducive to achieving more accurate early detection, improving screening efficiency, and reasonably allocating medical resources. Committee for Integrative Prevention and Screening of Breast Cancer, China Anti-Cancer Association takes the lead and organizes multi-disciplinary experts to systematically review relevant evidence-based medical evidence domestically and internationally, integrate the actual situation, and clinical experience of China, and conform to setting norms of international guidelines to complete evidence integration and form recommendations, thereby formulating “Expert Consensus on Breast Cancer Screening in Chinese Female with Dense Breast (2025 edition)”. This expert consensus puts forward recommendations on the risk stratification, screening age, screening interval, screening methods, joint screening strategies, and other key issues of females with dense breast, aiming to promote the standardized screening practice of such population, improve the level of early diagnosis and treatment of breast cancer, and provide technical support and implementation basis for the prevention and control of Chinese females with breast cancer.
In order to standardize the suppliers’ behavior and ensure the healthy development of medical and health services, West China Hospital of Sichuan University deeply analyzed the suppliers’ thought, behavior, and result risk under the idea of Integrity Risk Prevention and Control, and explored the management mode of " dare not rot” " can’t rot” and " don’t want to rot” from the suppliers’ perspective. Several methods were adopted to guide and control suppliers’ behaviors in a standardized way, such as system formulation and publicity, signing the " incorruptible purchasing and saling contract”, supplier filing, " sunshine promotion”, whole-process evaluation, serious accountability, etc. This model can provide a reference for the construction of a new type of cooperation relationship between hospitals and medical enterprises under the new situation.
ObjectiveTo investigate the strategy adjustment of breast cancer surgery management process during the outbreak of novel coronavirus pneumonia (NCP), and to summarize the experience.MethodsBased on "Pneumonitis Diagnosis and Treatment Plan for Novel Coronavirus Infection (Fifth Trial Version)" and "Surgery Work System During the Prevention and Control of Novel Coronavirus Infection in Mianyang Central Hospital (Second Edition)", the breast surgery department adjusted strategies and plans for breast cancer surgery during the epidemic.ResultsFrom January 25, 2020 to February 11, 2020, 8 cases of breast cancer surgery were performed in our hospital. They were all females, with an average age of 45 years. Five patients underwent modified radical mastectomy, 2 patients underwent breast-conserving surgery plus sentinel lymph nodes biopsy, and 1 case underwent breast-conserving surgery plus axillary lymph node dissection. All 8 patients were diagnosed as invasive ductal carcinoma of breast by hollow needle biopsy. All patients recovered smoothly after surgery, no complications occurred, and they were discharged as planned. No patients or medical staff showed NCP exposure and infection.ConclusionDuring the NCP epidemic, breast cancer surgery can be performed and the epidemic can be effectively prevented and controlled by adjusting the work flow and strategy.
In recent years, the prevalence of carbapenem-resistant Enterobacterales (CRE) has been remarkably increasing. Infections caused by CRE have significantly increased the burden on patients both medically and economically, and the CRE often leads to outbreaks of healthcare-associated infections. It has now become a global public health concern. Consequently, international organizations and academic societies/associations, including the World Health Organization, have developed corresponding prevention and control guidelines. This article provides a detailed introduction to the background, principles, key understandings, and implementation recommendations of China’s Standard for Prevention and Control of Carbapenem-resistant Enterobacterales (WS/T 826-2023). The aim is to enhance the recognition of healthcare workers and medical administrators to prevent and control CRE and to provide detailed technical guidance for healthcare facilities in responding to the spread of CRE, thus ensuring medical quality and patient safety.
Zhaojue is a deeply impoverished county located in Liangshan Autonomous Prefecture with high prevalence of human immunodeficiency virus infection. Based on local conditions, West China Hospital of Sichuan University has established a targeted supporting mode called “three-level organization and five-in-one”. This mode integrates administrative support teams, multiple disciplinary technical support teams and on-site expert teams to achieve five goals, including building effective teams, promoting clinical practice, enhancing skill training, focusing on academics and building a systematic platform. This model has improved the ability of treatment on human immunodeficiency virus infection and overall health services in Zhaojue County. It also reflects the responsibility of West China Hospital of Sichuan University in poverty alleviation and public welfare.
Carbapenemase producing Enterobacteriaceae (CPE) has emerged as a significant global public health challenge and placing infected patients at risk of potentially untreatable infections. When resistance to carbapenems occurs, there are often few alternative treatments available. Numerous international guidelines have performed systematic and evidence review to identify new strategies to prevent the entry and spread of CPE in healthcare settings. Several key strategies have been shown to be highly effective. Firstly a new strategy that is proven to be effective is the early identification of the CPE carrier patients through active surveillance cultures. While waiting for the screening results, suspected CPE carriers will be put on preemptive isolation in single room and healthcare worker will at the same time practice contact precautions. The active surveillance culture and prompt preemptive isolation will limit the entry and spread of CPE from getting into hospital. Secondly, it is of utmost importance to incorporate enforcement of the basic infection prevention and control best practices in the hospital including, full compliance to hand hygiene, appropriate use of personal protective equipment, execute antibiotic stewardship program to control abuse of antibiotics, effective environmental cleaning and decontamination, staff education and feedback, as well as surveillance of healthcare-associated infections. Such a holistic approach has been shown to be effective in inhibiting CPE from gaining foothold in the hospital.
Targeted initiatives of West China Hospital of Sichuan University to cope with coronavirus disease 2019 epidemic in three stages were summarized, including “three reconstructions” to mobilize in pre-epidemic stage, “three earlies” strategy to precisely treat critically ill patients in epidemic stage, and “three orderlies” to resume hospital operation in post-epidemic stage. The development of comprehensive hospitals in China after the epidemic was also discussed. It’s recommended to continuously strengthen the role of emergency response, early warning, comprehensive treatment, high-level talent training, international exchange, regional leadership, and collaborative innovation, in response to the shortcomings exposed in the epidemic.
Since the establishment in 2016, especially after the outbreak of coronavirus disease 2019 (COVID-19) epidemic, the Hospital Infection Management Quality Control Center of Shuangliu District has carried out quality management and technical ability training for full-time and part-time infection prevention and control personnel through various methods, and used grid management to organize examinations and assessments, inspections and supervision, as well as consultation and assistance. As a result, the culture of infection prevention and control has been deeply rooted in the hearts of the people, the health administrative departments have increased investment in infrastructure and equipment, the funds for infection prevention and control have continued to increase, and the prevention and control quality of COVID-19 and daily works in the region has been continuously improved. The center plays a leading role in the promotion of the standardization, homogenization, and quality improvement of the prevention and control of hospital infection in the district.
