        • The application and prospect of intestinal ultrasound in patients with Crohn’s disease

Crohn’s disease (CD) is a chronic, remitting and segmental inflammatory disease of the gastrointestinal tract, radiological examinations play critical roles in the diagnosis of CD. The common radiological examinations include ultrasound examinations of intestine, computed tomography enterography (CTE), and magnetic resonance enterography (MRE) in clinic. The application of CTE and MRE is limited because of radiation exposure or higher costs. The ultrasound examinations of intestine including conventional intestinal ultrasound, ultrasonic elastography, and contrast-enhanced ultrasound have been studied because of the advantages of non-invasiveness and convenience. The accuracy of the intestinal ultrasound has been already partially validated in assisting diagnosis and evaluating the disease activity and location in CD patients. This review aims at summarizing the diagnostic potency and prospect of the application of ultrasound examinations of intestine in CD patients.

        • A CLINICAL PATHOLOGIC OBSERVATION OF 19 CASES OF COATS DISEASE

OBJECTIVE:Observing the clinical and pathological features of Coats disease. METHODS:Reviewing the clinical data and pathologic slides duly confirmed by pathology of 19 cases of Coats disease, which belonging to our college's Laboratory of Ophthalmologic Pathology from 1959 to 1994. RESULTS: 14 males,5 females, aged 1-18 years. More boys were affected than girls in the age group under 10 and that difference between both sexes became gradually less as they grew older. The main pathologic changes were the vascular dilatation and congestion of the outer layer of the retina, the uneven thickness of the vascular walls and the proliferation of the connective tissue. Retinal protuberance was seen in most of the advanced cases.with bleeding and vascular changes on its surfaces. The main pathologic changes were the detachment of retina and the appearance of many foam cells and crystals of cholesterol in the subretnal fluid, and calcification and ossification of the outer layer of the retina were found in some cases. CONCLUSION :Cytological examination of the subretinal fluid might be the liable method in differentiating between the Coats disease and retinoblatstoma. (Chin J Ocul Fundus Dis,1996,12: 157-159)

        • A scoping review of clinical research evidence for traditional Chinese medicine in the treatment of coronavirus disease 2019

Objective To conduct a scoping review on the clinical research evidence for the treatment of coronavirus disease 2019 (COVID-19) with traditional Chinese medicine, identify relevant problems in the literature, and provide ideas for the follow-up research. Methods PubMed, Embase, the Cochrane Library, China National Knowledge Infrastructure, Wanfang Digital Journal Full-text Database, and China Biomedical Literature Database were searched from inception to July 21st, 2022. The clinical research evidence for the treatment of COVID-19 with traditional Chinese medicine was included, the data information was sorted out, and the results were descriptively analyzed. Results A total of 132 studies were included, including 53 randomized controlled trials, 17 non-randomized controlled trials, and 62 retrospective cohort studies, all of which were published between 2020 and 2022. The clinical studies were carried out in 19 provincial level regions, among which Hubei province had the largest number of studies (49.2%, 65/132). The sample sizes of the studies were mostly between 50 and 100 cases (43.2%, 57/132). Most of the studies had a treatment course of 0-14 days (50.0%, 66/132). The most compared intervention measures were traditional Chinese medicine + conventional western medicine treatment vs. conventional western medicine treatment, accounting for 75.0% (99/132) of the studies. The COVID-19 patients included in the studies were mainly mild and moderate. Outcome indicators included changes in symptoms/signs, laboratory indicators, CT indicators, clinical outcomes, safety indicators, functional scales, etc. The main adverse reactions/events in intervention/exposure groups were gastrointestinal reactions. Conclusions There has been a lot of clinical research evidence on the treatment of COVID-19 by traditional Chinese medicine. To provide strong evidence support for the treatment of COVID-19 by traditional Chinese medicine, more clinical trials with large samples and international collaboration are needed in the future.

        • Assessment of Combined Management for Lower Limb Chronic Venous Insufficiency According to CEAP Classification

Objective To assess the combined management of lower limb chronic venous diseases according to the CEAP classification. Methods One hundred and twenty patients were classified according to the CEAP classification. Based on clinical presentation and image study, all patients were treated with combined management plan including oppression, medication and surgery. Results All 120 patients (135 limbs) were followed up in clinic, the local recurrence rate was 18.52%(25/135). Conclusion CEAP classification expounds the developing process of lower limb chronic venous diseases. With CEAP, we can avoid the blind spot in the treatment and expand the extent of combined therapy. Accordingly, CEAP classification is useful in the treatment and diagnosis of chronic venous diseases.

        • Clinical characteristics of Menkes disease

ObjectiveTo improve the understanding of clinicians by reports and literature review of patients with rare diseases of Menkes disease.MethodsHigh-throughput sequencing and Sanger sequencing were used to verify the genes of epilepsy, and the mutations were verified in three probands and two parents. The patient's clinical manifestations, EEG, imaging, gene and prognostic characteristics were analyzed.ResultsAll the three patients developed onset in infancy, with hair thinning and curling, and various forms of seizures. Three patients had epileptiform discharges during the EEG interval, and all clinical seizures were recorded. Skull MR showed white matter long T1, long T2 abnormal signal, cerebral artery tortuosity, proband 3 appeared subdural effusion. Three patients had poor efficacy after taking anti-epileptic drugs. The proband one and the proband two did not show significant progress after using histidine copper, but could not alleviate the existing neurological damage.ConclusionMenkes disease occurs frequently in infants, clinical manifestations may be different, some clinical manifestations may be atypical, and currently it is an incurable disease, but the use of histidine copper in the neonatal period can improve survival and reduce nervous system injury. It should be diagnosed early. and the treatment of indications should not be guided by the patient's genotype.

        • A new method for classification of Alzheimer’s disease combined with structural magnetic resonance imaging texture features

In this paper, a new method for the classification of Alzheimer’s disease (AD) using multi-feature combination of structural magnetic resonance imaging is proposed. Firstly, hippocampal segmentation and cortical thickness and volume measurement were performed using FreeSurfer software. Then, histogram, gradient, length of gray level co-occurrence matrix and run-length matrix were used to extract the three-dimensional (3D) texture features of the hippocampus, and the parameters with significant differences between AD, MCI and NC groups were selected for correlation study with MMSE score. Finally, AD, MCI and NC are classified and identified by the extreme learning machine. The results show that texture features can provide better classification results than volume features on both left and right sides. The feature parameters with complementary texture, volume and cortical thickness had higher classification recognition rate, and the classification accuracy of the right side (100%) was higher than that of the left side (91.667%). The results showed that 3D texture analysis could reflect the pathological changes of hippocampal structures of AD and MCI patients, and combined with multi-feature analysis, it could better reflect the essential differences between AD and MCI cognitive impairment, which was more conducive to clinical differential diagnosis.

        • Optimizing adalimumab therapies using therapeutic drug monitoring: current evidence and future perspectives

Therapeutic drug monitoring (TDM) has been more widely used in small molecule agents, such as immuno-suppressants, antiepileptic drugs and antibiotics, with less attention in the field of therapeutic biological agents. Monoclonal drugs represented by tumor necrosis factor alpha (TNF-α) inhibitors have shown a good relationship between exposure and efficacy in clinical studies. There are corresponding guidelines and consensus for the recommendations of TDM based on current research evidence. Therefore, this paper introduced the current evidence, strategies and considerations for TDM in the optimal treatment of adalimumab from the perspective of adalimumab TDM to provide references for the clinical practice of adalimumab TDM.
