目的了解德陽地區妊娠婦女甲狀腺功能（甲功）異常的發病率及甲狀腺自身抗體的陽性率，為妊娠期甲功異常的早期診斷和干預提供依據。方法選取2014年3月－9月在德陽市人民醫院產科門診就診的妊娠婦女2 062例作為研究對象，采用電化學發光免疫分析技術檢測促甲狀腺素（TSH）、游離甲狀腺素（FT4）及抗甲狀腺過氧化物酶抗體（TPOAb），根據甲功指標分為甲功異常組和甲功正常組。結果 2 062例妊娠婦女中，甲功異常組688例，占33.4%，分別為亞臨床甲狀腺功能減低（甲減）511例（24.78%），臨床甲減114例（5.53%），亞臨床甲狀腺功能亢進（甲亢）38例（1.84%），臨床甲亢25例（1.21%）。甲功異常組和甲功正常組TPOAb陽性率分別為17.73%（122/688）、8.73%（120/1 374），差異有統計學意義（χ²=30.839，P＜0.001）。按臨床甲減、亞臨床甲減、亞臨床甲亢、臨床甲亢的順序TPOAb陽性率有增高的趨勢（χ²_趨勢=11.600，P=0.001）。結論德陽地區妊娠婦女甲功異常發生率較高，主要為亞臨床甲減，較多合并自身抗體異常。將甲功及抗體檢測納入孕期檢查，有利于減少甲狀腺疾病對孕婦和胎兒的危害。
