目的總結 Ilizarov 技術分期治療合并嚴重肢體短縮畸形的重度先天性脛骨假關節（congenital pseudarthrosis of tibia，CPT）的療效。方法 2014 年 5 月—2018 年 11 月，應用 Ilizarov 技術分期治療 5 例重度 CPT 患兒。男 1 例，女 4 例；年齡 2 歲 6 個月～7 歲 1 個月，中位年齡 4 歲 7 個月。術前患側脛骨長度較健側短縮 4.0～6.8 cm，平均 6.06 cm。按照 Boyd 分型：Ⅰ型 1 例、Ⅱ型 3 例、Ⅴ型 1 例。一期切除脛骨假關節及病損組織，應用 Ilizarov 技術行肢體延長，矯正肢體短縮畸形；二期行骨搬移至端端對合后，行植骨內固定促進對合端愈合。結果 5 例患兒均獲隨訪，隨訪時間 12～60 個月，平均 41 個月。外固定架佩戴時間 210～360 d，平均 262 d。肢體延長 4.5～8.0 cm，平均 6.06 cm；患肢脛骨與健側等長或略短（≤2 cm）。術后 7～12 個月脛骨假關節均骨性愈合；愈合指數 42～50 d/cm，平均 45.2 d/cm。均無血管神經損傷、固定針折斷及不愈合等并發癥發生。結論應用 Ilizarov 技術分期治療重度 CPT，可矯正肢體短縮畸形、減少肌腱攣縮、創傷小、脛骨對合端穩定性好、愈合率高、再骨折發生率低，可最大限度改善肢體功能。
