目的探討尖端賽多孢子菌致免疫正常宿主肺部感染的病因及發病機制、臨床表現、診斷及治療，通過對相關文獻的復習，提高臨床對此病的認識，減少誤診、漏診。方法對 1 例尖端賽多孢子菌致肺部感染患者的臨床表現、實驗室檢查、影像學檢查及治療進行報道并結合相關文獻分析。結果患者女性，47 歲，因“痰中帶血 1⁺個月”入院。胸部 CT 提示右肺下葉背段支氣管擴張伴周圍少許感染，擴張支氣管腔內結節影。肺泡灌洗液病原微生物高通量基因檢測結果提示尖端賽多孢子菌。結合患者臨床表現擬診為肺尖端賽多孢子菌病，予伏立康唑抗真菌感染治療后患者癥狀緩解。文獻復習共檢索到尖端賽多孢子菌病病例報道 1000 余例，其中致免疫功能正常者肺部感染僅 40 余例。多為散發報道或小樣本報道。結論尖端賽多孢子菌致免疫正常宿主肺部感染相對罕見，此真菌侵襲力強，感染部位廣，預后差，臨床上應提高警惕，盡早診斷。治療上依賴于抗真菌治療、免疫調節治療和在某些情況下手術切除的聯合治療。
