Objective To explore the feasibility and safety of Frey operation under laparoscopy in the treatment of chronic pancreatitis. Methods The clinical data of chronic pancreatitis patients who underwent laparoscopic Frey procedure in Division of Pancreatic Surgery, Department of General Surgery, West China Hospital, Sichuan University from 2021-2023 were retrospectively analyzed. Results Eleven patients with chronic pancreatitis complicated with pancreatic duct stones were included in the study, 7 of whom had a history of diabetes and (or ) impaired glucose tolerance, suggesting that pancreatic endocrine function was impaired. The median diameter of the main pancreatic duct measured by imaging method was 8 mm (4–20mm). The median operative time was 188 min (120–368 minutes), and the total intraoperative bleeding volume was 50 mL (20–100 mL). Postoperative pancreatic fistula did not occur, one case of postoperative abdominal fluid accumulation and hypoproteinemia improved after symptomatic supportive treatment such as anti infection, acid and enzyme inhibition, and nutritional rehydration. Postoperative bleeding occurred in 3 cases, including 1 case of intestinal anastomotic bleeding, which was sutured again under emergency laparoscopic intestinal anastomosis to stop bleeding. The other two cases improved after conservative management such as blood transfusion, plasma, vitamin K, acid inhibition, enzyme inhibition and hemostatic drugs. The median postoperative hospitalization time was 7 days (4–18 days), and no patient mortality happened within 90 days after surgery. Conclusion Laparoscopic Frey operation is feasible and a relatively safe and effective method for the treatment of chronic pancreatitis.
Objective To analyze the quantitative relationship between respiratory effort and inferior vena cava (IVC) diameter variability in healthy adults, and explore the effects of respiratory effort on the fluid responsiveness with IVC diameter variability. Methods From October 2022 to May 2023, a cross-sectional study was conducted in healthy young subjects who met the criteria. Respiratory effort was evaluated by using portable pulmonary function to measure the subjects’ inspiratory conditions in three states (quiet breathing, moderate inspiration, and maximal inspiration). At the same time, the IVC internal diameter was measured by bedside ultrasound and the IVC diameter variability was calculated. The correlation between inspiratory volume and IVC diameter variation was analyzed, and the receiver operator characteristic (ROC) curve was drawn. The sensitivity and specificity of fluid responsiveness induced by inspiratory effort were predicted according to the area under the ROC curve (AUC). Results A total of 95 subjects were screened, aged 27.13±5.77 years, of whom 30 (32%) subjects were males. During quiet breathing, 41.1% of subjects had IVC inner diameter variation ≥50%. For moderate inspiration, it was 68.4%. At maximum inspiration, this proportion is more than 85%. Inspiratory volume was moderately positively correlated with IVC diameter variation, and the correlation coefficient r=0.45. With the IVC diameter variation ≥50% as the positive criterion for fluid responsiveness, the AUC of fluid responsiveness induced by inspiratory effort was 0.73 (95% confidence interval 0.67 - 0.78, P<0.001), and the inspiratory volume threshold was 13 mL/kg ideal body weight when the maximum Youden index was 0.41. That is, moderate force breathing can induce fluid responsiveness, with sensitivity of 79.57% and specificity of 61.62%. Conclusion The degree of respiratory effort significantly affects the IVC inner diameter variation, and there may be false positives in the evaluation of fluid responsiveness according to IVC inner diameter variation in the case of spontaneous breathing.
Objective
To investigate the safety of high fraction of inspired oxygen (FiO2)during noninvasive ventilation in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD)and carbon dioxide (CO2)retention.
Methods
Fifty-six AECOPD patients with CO2 retention admitted between March 2013 and August 2015 were recruited in the study.All patients received noninvasive ventilation treatment with FiO2<0.5.After stabilization of acute respiratory crisis,FiO2 was increased to 1.0 and lasted for 40 minutes.The changes of tidal volume,respiratory frequency,minute volume,Glasgow coma score,arterial blood gas and SpO2 were observed before and after the FiO2 reset.
Results
The mean PaO2 increased from (83±14)mm Hg to (165±41)mm Hg and the mean SpO2 increased from (92.4±3.1)% to (97.8±1.9)% significantly (both P<0.001).The mean PaCO2 did not changed obviously from (72±15)mm Hg to (72±14)mm Hg (P=0.438).There were also no significant changes in any of the other parameters.
Conclusion
During noninvasive ventilation with an FiO2 sufficient to maintain a normal PaO2,an increase in FiO2 does not further increase PaCO2 level in AECOPD patients with CO2 retention.
ObjectiveTo investigate the feasibility of electroencephalography (EEG) power spectrum analysis monitoring noninvasive intracranial pressure (ICP).
MethodsBetween September 2008 and May 2009, the EEG signals were recorded in 62 patients (70 cases/times) with central nervous system (CNS). By using self-designed software, EEG power spectrum analysis was conducted and pressure index (PI) was calculated automatically. ICP was measured by lumbar puncture (LP).
ResultsThe mean ICP was (239.74±116.25) mm H2O (70-500 mm H2O, 1 mm H2O=0.009 8 kPa), and 52.9% of patients had increased ICP. The mean PI was 0.29±0.20 (0.02-0.85). The Spearman rank test showed that there was a significant negative correlation between PI and ICP (rs=-0.849, P<0.01). The data from the patients with diffuse lesions of CNS and focal lesions were analyzed separately; the results showed there were significant negative correlations between PI and ICP in both groups (rs=-0.815, -0.912; P<0.01).
ConclusionThe PI obtained from EEG analysis is correlated with ICP. Analysis of specific parameters from EEG power spectrum might reflect the ICP. Further research should be carried out.
ObjectiveTo evaluate the feasibility and safety of laparoscopic radical antegrade modular pancreatosplenectomy through the ligament of Treitz approach for pancreatic body and tail cancer. MethodsOn 13th November 2017, we selected a patient with a suspicious malignant tumor in the body of pancreas but no evidence of metastasis or local invasion of the retroperitoneum for laparoscopic antegrade modular pancreatosplenectomy through the ligament of Treitz approach. The time of operation, the estimated blood loss, and post-operative complications were observed. ResultsThe procedure was completed successfully in 255 min, and the estimated blood loss was 200 mL, there was no need of transfusion and no significant post-operational complications had been observed. No tumor recurrence or distal metastasis was found after a 12 month’s follow-up. ConclusionLaparoscopic radical antegrade modular pancreatosplenectomy through the ligament of Treitz approach is a feasible and safe procedure for pancreatic body and tail malignant tumor in strictly selected patients.
ObjectiveTo evaluate hepatic venous Doppler parameters under varying inspiratory effort levels in healthy adults using ultrasound, providing a theoretical basis for assessing the impact of inspiratory effort on hepatic venous blood return. MethodsA cross-sectional study was conducted from December 2024 to March 2025, enrolling 61 healthy adults. Inspiratory effort was quantified via portable pulmonary function testing, measuring inspiratory volume (IV) during quiet breathing, moderate inspiratory effort, and maximal inspiratory effort. Hepatic venous Doppler parameters (a-wave, s-wave, d-wave peak velocities) were assessed bedside under these three conditions. Correlation analyses and linear regression models were used to explore relationships between IV and hepatic venous flow parameters. ResultsA total of 61 participants completed screening, with a mean age of 27.03 ± 4.73 years (24 males, 39.0%). Although male participants had advantages in height and weight, no significant differences in inspiratory volume (IV), hepatic venous a-wave, s-wave, d-wave peak velocities, or s/d ratio were observed between men and women across all breathing states (all P>0.05). Significant negative correlations were identified between IV and a-wave (r=?0.68, 95%CI: ?0.75 to ?0.59), s-wave (r=?0.56, 95%CI: ?0.65 to ?0.45), and d-wave (r=?0.33, 95%CI: ?0.46 to ?0.20) velocities. Linear regression equations were established as follows:a-wave (cm/s) = 22.06 - 0.23 × IV (ml/kg ideal body weight); s-wave (cm/s) = 34.96 - 0.26 × IV (ml/kg ideal body weight); d-wave (cm/s) = 25.82 - 0.11 × IV (ml/kg ideal body weight).The s/d ratio decreased significantly with increasing inspiratory effort (1.37 ± 0.38 vs. 1.23 ± 0.24 vs. 1.15 ± 0.22; F=9.91, P<0.001). However, only a weak negative correlation (r=?0.28, P<0.001) was observed between inspiratory effort and s/d ratio. ConclusionInspiratory effort demonstrates a strong negative correlation with hepatic venous blood peak flow velocities in healthy adults. While the s/d ratio shows a downward trend with deeper inspiration, the association is weak.
