        • EXPRESSION AND ROLE OF METALLOPROTEINASES IN BREAST CANCER

Objective To study the relationship between metalloproteinases (MMPs) and breast cancer. Methods The literature in recent years on the relationship between the expression of MMPs and breast cancer was reviewed. Results The balance between MMPs and tissue inhibitors of metalloproteinases (TIMPs) is keeping normally kept in human body. Many of the studies showed that the expression of MMPs is increased in breast cancer. Conclusion The growth, invasion and metastasis of breast cancer is closely related with the increased expression of MMPs. This suggests that MMPs is a valuable prognostic marker and TIMPs would be a novel drug against cancer.

        • Disease burden and attributable risk factors of breast cancer in Chinese females from 1990 to 2019

ObjectiveTo analyze the latest epidemiological status of breast cancer in China, trends in morbidity and mortality from 1990 to 2019, and related prognostic risk factors.MethodsData on incidence and mortality of Chinese female breast cancer, their related age-standardized rates (ASRs) from 1990 to 2019, and attributable risk factors were obtained from the Global Burden of Disease (GBD) database, and data on disability-adjusted life years (DALYs) of 34 provinces in China were obtained from literature. Joinpoint regression analysis was used to analyze the trends of ASRs. The exposure levels of each attributable risk factor and the increased cancer burden were analyzed.ResultsThe incidence of breast cancer in Chinese females increased annually, from 17.07/100 000 in 1990 to 35.61/100 000 in 2019, while the mortality rate initially increased and decreased, and then exhibited an upward trend after 2016 and there was no obvious variation from 1990 (9.16/100 000) to 2019 (9.02/100 000). Among the 34 provinces of China, Shandong Province had the most serious breast cancer burden, while Macao Special Administrative Region had the lowest. Among the seven prognostic risk factors, high body mass index (BMI) contributed the most to the breast cancer burden and the exposure risk of a diet high in red meat had shown a significant increasing trend in the past 30 years. Therefore, the disease burden caused by a high red meat diet would be increasing.ConclusionsThe incidence rate of breast cancer in Chinese females is increasing. With the development of social economy and the change of people’s dietary habits, the breast cancer burden in China trends to become heavier and heavier. Therefore, it is necessary to conduct the "three early" prevention and treatment and advocate healthy and reasonable diet and living habits to reduce the burden of breast cancer to improve prognosis and quality of life.

        • Effectiveness of Self-Management in Breast Cancer Patients: A Systematic Review

Objective To systematically assess the effectiveness of self-management during adjuvant treatment for breast cancer. Methods Through formulating the inclusion and exclusion criteria and the search strategy, such databases as The Cochrane Library, PubMed (1966 to 2010), OVID, ELSEVIER, CINAHL, EMbase, CNKI (1980 to 2010), WanFang Data, VIP (1989 to 2010) and CBM (1978 to 2010) were searched to collect the randomized controlled trials (RCTs) about postoperative self-management in breast cancer. Two reviewers evaluated the quality of the included studies, extracted and crosschecked the data; the Review Manager 5.0 software was used to perform the test for heterogeneity, and the cumulative effect was calculated with either fixed or random effects models. Results Among the included ten studies in English, three revealed that the program of self-management had no impact on the overall life quality of patients, three revealed that it could enhance the self-care ability of patients (P=0.004), three revealed that it could not reduce the anxiety level of patients, and two revealed that it had certain influence on the depression level of patients. Conclusion The program of self-management can enhance the patients’ self-care ability, but its influence on patients’ overall life quality as well as the level of anxiety and depression is still undefined.

        • SKIN SPARING MASTECTOMY AND IMMEDIATE BREAST RECONSTRUCTION

Objective To investigate the results of skinsparing mastectomy and immediate breast reconstruction with transverse rectus abdomins musculotaneous(TRAM) flap or latissimus dorsi musculocutaneousflap plus placement of a mammary implant. Methods From June 1997 to June 2002, 11 patients were proven to have ductal carcinoma in situor huge breast carcinoid by pathological examination. The site of the biopsy incision was around the areola. The patients underwent mastectomy with skin sparing by a circumareolar incision and immediate breast reconstruction withTRAM flap or latissimus dorsi musculocutaneous flap plus placement of mammary implant.Autogenous tissue was used to fill the skin envelop. The second stage operation of nipple-areola reconstruction was performed on the replaced skin. Results Eleven patients were followed up 1 month to 6 years. The operative result was good and all patients had no relapse. The reconstructed breast achieved good results in shape, colour, sensation, symmetry and incision scar. Conclusion The skin sparing mastectomy and immediate autograft tissue breast reconstruction is an ideal reconstructive method for the patients with breast ductal carcinoma in situ or huge breast carcinoid in condition that there were strict operative indication and relapse can be prevented.

        • Relationship of Topoisomerase TypeⅡA to Anthracycline in Breast Cancer

ObjectiveTo explore the relationship of topoisomerase typeⅡA (TOP2A)to human epidermal growth factor receptor 2(HER2/neu), and the therapeutic prediction of TOP2A in clinical chemotherapy for breast cancer. MethodThe Cochrane, Medline, Embase, PubMed, CNKI, and WANFANG data were retrieved to get the related literatures about TOP2A and the effect of anthracycline-based drugs. ResultsThe TOP2A gene and HER2 gene were all located in 17q21, the TOP2A gene was a downstream gene of the HER2 gene. There was some relationship of the HER2 to TOP2A gene abnormality. There was some controversy about HER2 and TOP2A on anthracycline regimens reaction. It was generally accepted that the status of TOP2A gene for predicting the effect of patients with anthracycline regimens might be more precise. There was inconsistency between the gene amplification and overexpression of TOP2A, which was two different biological behaviors, and which might be the characteristics of different molecular subtypes. In terms of the present study, a plenty of clinical researches could illustrate that TOP2A was an important target of anthracycline. But anthracycline-based drugs could induce the apoptosis of the cell through various mechanisms, so the anthracyclinebased drugs killed tumor cells in breast cancer needed not inhibit TOP2A, might be more complex than previously predicted. ConclusionsTOP2A is an important target of anthracycline. But it seems to be limited only by observing the TOP2A for predicting curative effect of anthracycline-based drugs.

        • Relationship Between Estrogen Receptor α Gene PvuⅡ, XbaⅠPolymorphism and Breast Hyperplasia

ObjectiveTo investigate the relationship between the polymorphisms of estrogen receptor α (ERα) gene PvuⅡ, XbaⅠ and breast hyperplasia. MethodsPolymerase chain reaction-restriction fragment length polymorphism was used to detect the polymorphisms of ERα gene PvuⅡ, XbaⅠ in breast hyperplasia patients (study group, n=89) and healthy controls (control group, n=35). ResultsThe differences of the genotypic frequency and allele frequency of the ERα gene Xba Ⅰ were significant between the study group and the control group (Plt;0.05). According to analysis of the odds ratio (OR), the risk of developing breast hyperplasia for X allele carriers was 0.551 as compared with x allele carriers. But there was no significant difference for the gene polymorphism of PvuⅡ between the study group and the control group (Pgt;0.05). ConclusionThe polymorphisms of XbaⅠof ERα gene is associated with breast hyperplasia and the mutant gene increases breast hyperplasia risk.
