        • Thoughts on prevention and management strategies for surgical site infection

          Surgical site infection (SSI) is a common hospital acquired infection that can increase medical burden and affect patient prognosis. Its occurrence involves multiple factors such as the patient’s basic condition and perioperative management quality. Although there is a basic consensus on SSI prevention in domestic and foreign guidelines, there are still differences between the recommendations in the guidelines and infection prevention and control management. To further promote the implementation of the guidelines, this article reviews the key preventive measures for SSI in domestic and foreign guidelines from preoperative skin preparation, intraoperative standardized operation, and postoperative incision management, and explores in depth the management strategies of SSI, in order to provide a reference for building a full process infection prevention and control system for SSI.

        • Numerical simulation study on the influence of free edge configuration on the performance of polymeric heart valves

          This study aims to investigate the effects of curved and straight free edges on the hemodynamic performance and mechanical properties of polymeric heart valves. Two aortic valve models with different free-edge configurations were established, and valve motion throughout the entire cardiac cycle was simulated using a two-way fluid-structure interaction (FSI) method. Hemodynamic parameters and stress distribution characteristics were compared and analyzed. The results revealed that the curved free edge valve exhibited a significantly faster opening response than the straight free edge valve, with an approximately 13% increase in the effective orifice area (EOA) and an approximately 27% reduction in regurgitant volume. However, after valve closure, the curved free-edge model demonstrated higher stress levels across all critical regions. The free-edge configuration did not significantly alter the vortical structure within the aortic flow field; both models exhibited a flow pattern characterized by a combination of sinus vortices and wall-mounted spindle-shaped vortices. The findings indicate that a curved free edge can improve valve opening efficiency and regurgitation control, but may exacerbate stress concentration during closure, potentially increasing the risk of fatigue damage to the valve.

        • Experience of Da Vinci Robotics-Assisted Dixon Procedure for Rectal Cancer in 11 Cases

          Objective To summary the early experience of Dixon procedures with Da Vinci robotics surgical system for rectal cancer. Methods Eleven patients with rectal cancer underwent the combination of laparoscope and Da Vinci robotics surgical system with 4 trocars in our hospital from May. 2011 to Jan. 2012. Laparoscopy was firstly used to identify the possibility of the surgical procedure, then placed the 4 trocars, and maked sure the suspension of the sigmoid colon and the uterus. Transections of rectum were performed by a conventional laparoscopic method, and endoscopic separations were performed by Da Vinci robotics surgical system. The clinical data were retrospectively analyzed and the experience was summarized. Results The Da Vinci robotics-assisted Dixon procedures were successfully performed in 11 patients and no one turned to laparotomy. The operating time was 210-330min (mean 288.6min);the blood loss was 20-100ml (mean 40ml); The number of lymph nodes dissected was 12-21 per case (mean 13.9 per case);the duration of bowel movement and hospital stay were 18-26h (mean 22h) and 7-16d (mean 11.5d), respectively. There were no intraoperative or postoperative complications related to the use of robotics, and no residual cancer cells at resection margin. Conclusions Da Vinci robotics-assisted Dixion procedure with 4 trocars and suspension of sigmoid colon are safe and feasible, and it is beneficial to the recovery of patients
