        • Efficacy and long-term retention rate comparision of ketogenic diet (KD) for children with intractable epilepsy in outpatient department and inpatient department

ObjectiveTo compare the efficacy and compliance of children children with refractory epilepsy receiving ketogenic diet (KD) in outpatient department with children receiving KD treatment in inpatient department. MethodsA retrospective study of 44 children with intractable epilepsy receiving the modified classical ketogenic diets in outpatient department from June 2014 to December 2015, who were followed-up during the third, sixth and twelfth month. Records of epileptic seizures and adverse reactions were used to evaluate the efficacy and retention rate of inpatient department KD treatment in children with refractory epilepsy, and compared with 104 children receiving KD treatment in inpatient department at the same period. ResultsThirty-four of the forty-four children comleted observation after 12-month follow-up, 15 cases had been seizure freedom, 22 cases had more than 50% reduction in seizure frequency, 12 patients had less than 50% reduction in seizure frequency. The total effective rate of the KD therapy in outpatient department was 64.7%, and the retention rate was 71%. 18 of of the 104 children with KD treatment in inpatient department at the same period comleted observation after 12-month follow-up, 3 cases had been seizure freedom, 5 cases had more than 50% reduction in seizure frequency, 13 cases had less than 50% reduction in seizure frequency. The total effective rate of the KD therapy in inpatient department was 27.8%, and the retention rate was 17.3%. ConclusionThe KD therapy in outpatient department is effective to children with intractable epilepsy, and there is a highly efficacy and compliance of children receiving KD in outpatient department comparing with children receiving KD in inpatient department. MethodsPrescription of antibiotic drugs were analyzed by a retrospective comparative analysis during April to June 2011, August to October 2011 and August to October 2012. ResultsPrescriptions of antibiotic drugs were 435 (36.25%), 367 (30.58%) and 223 (18.58%) during April to June 2011, August to October 2011 and August to October 2012, respectively, and the unreasonable prescriptions were 35 (2.92%), 27 (2.25%), and 14 (1.25%) respectively. ConclusionThere is a decreased tendency of prescription of antibiotic drugs in the outpatient department of our hospital because of the special rectification activities of antibiotic drugs. It is in accordance with antibiotic use rate of <20% target of "antibiotics in 2012 special rectification program" in hospital outpatient service. But there are some unreasonable phenomena in the use of antibiotics, and to promote the reasonable use of antibiotics, we should strengthen management.

        • Study on the service intervention of medication therapy management for outpatient children with epilepsy

Objective To explore the service contents of medication therapy management (MTM) for outpatient epileptic children by analyzing drug related problems (DRPs). Methods A cross-sectional study was adopted to analyze the status of DRPs in outpatient epileptic children in West China Second Hospital of Sichuan University. The focus group discussion method was adopted to formulate the specific service contents of MTM in outpatient epileptic children. Results A total of 2 754 cases of antiepileptic drug treatment were received in the pediatric clinic of West China Second Hospital of Sichuan University from October 1st, 2018 to December 31st, 2018, including 2 018 cases of monotherapy and 736 cases of combination therapy, involving 7 drugs and 10 specifications. The specifications in descending order of frequency of drug use were levetiracetam oral solution, levetiracetam tablets, sodium valproate oral solution, oxcarbazepine oral suspension, oxcarbazepine tablets, topiramate capsules, sodium valproate sustained-release tablets, lamotrigine tablets, magnesium valproate sustained-release tablets and carbamazepine tablets. We applied the Pharmaceutical Care Network Europe (PCNE) classification and found 718 DRPs, mainly focusing on drug selection and improper course of treatment. The specific service contents of MTM for children with epilepsy in the clinic included hospital pharmacy information services, medical teams established by pharmacists, doctors and nurses, child education and medication consultation, and medication follow-up for discharged children. In addition, specific measures to solve DRPs in outpatient epileptic children were designed from 2 aspects of disease management and medication education, and 3 aspects of drug prescription, dispensing, and usage. Conclusions There are various types of antiepileptic medications and treatment schemes for children, and DRPs are complicated. Therefore, MTM for children with epilepsy requires to be strengthened. In this study, the specific intervention contents of MTM for outpatient epileptic children are designed to carry out the follow-up empirical study to verify the effectiveness of MTM.

        • Application of autoregressive integrated moving average model in prediction of outpatient and emergency visits in a district-level public hospital

Objective To accurately predict the outpatient and emergency visits of a district-level public hospital based on autoregressive integrated moving average (ARIMA) model, providing important basis for hospital budget planning and operational decisions. Methods The monthly outpatient and emergency visits of a public hospital in Shuangliu District, Chengdu City from January 2012 to November 2023 were collected, and R 4.3.1 software was used to establish an ARIMA model based on the data from January 2012 to December 2022. The outpatient and emergency visits from January to November 2023 were predicted and validated. Results Except for January and March 2023, every monthly number of predicted outpatient and emergency visits for 2023 matched the actual one relatively well. The average absolute percentage error for January to November 2023 was 8.504%. The actual total number of outpatient and emergency visits from January to November 2023 was 1441960, and the predicted value was 1417130 with a relative error of –1.722%. Conclusions ARIMA model can predict the outpatient and emergency visits of district-level hospitals relatively well. However, factors such as the high incidence of COVID-19 may affect the accuracy of short-term prediction.
