目的總結微創附加鋼板治療下肢長骨骨折髓內釘固定術后肥大性骨不連的可行性和效果。方法回顧分析 2010 年 1 月—2015 年 8 月采用微創附加鋼板治療的 11 例下肢長骨骨折髓內釘固定術后肥大性骨不連患者臨床資料。男 8 例，女 3 例；年齡 18～63 歲，平均 34.6 歲。股骨 8 例，脛骨 3 例。原始骨折國際內固定研究協會（AO/ASIF）分型：A 型 5 例，B 型 4 例，C 型 2 例。髓內釘固定手術至本次手術時間為 6～22 個月，平均 12.4 個月。術前患者膝關節或踝關節活動度（range of motion，ROM）較對側減少（30.71±14.72）°。結果手術切口均 Ⅰ 期愈合。11 例患者均獲隨訪，隨訪時間 12～30 個月，平均 14 個月。X 線片示骨折均愈合，愈合時間 4～8 個月，平均 5.7 個月。無感染、神經血管損傷、鋼板髓內釘斷裂等并發癥發生。末次隨訪時，患膝關節或踝關節 ROM 較對側減少（15.36±12.17）°，與術前比較差異有統計學意義（t=5.571，P=0.000）。患肢功能恢復按 Schatzker-Lambert 股骨遠端骨折功能評分或 Kaikkonen 踝關節損傷功能評分分級法，優 5 例，良 5 例，可 1 例，優良率 90.9%。結論微創附加鋼板治療下肢長骨骨折髓內釘固定術后肥大性骨不連，方法簡便，安全可行，效果良好。
