        • Early screening and evaluation of sepsis in emergency department

          Sepsis is a critical condition. The key factor affecting the survival of patient is whether standard treatment can be obtained timely. Because of the complexity of its pathogenesis and high heterogeneity, there is no special diagnosis method currently. Early identification is difficult. Delayed diagnosis and treatment is closely related to the mortality of patients. With the continuous updating of the guidelines, sepsis has been included in the “time window” disease, putting forward a great challenge to the early screening and evaluation of sepsis. This article aims to review the application of Sepsis-Related Organ Failure Assessment, sepsis biomarkers and artificial intelligence algorithms in early screening and evaluation of sepsis, so as to provide guidance tools for timely starting standardized treatment of sepsis.

        • The interpretation of the 2017 American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care: Adult Basic Life Support and Cardiopulmonary Resuscitation Quality

          The American Heart Association (AHA) released the 2017 American Heart Association Focused Update on Adult Basic Life Support and Cardiopulmonary Resuscitation Quality (2017 AHA guidelines update) in November 2017. The 2017 AHA guidelines update was updated according to the rules named " the update of the guideline is no longer released every five years, but whenever new evidence is available” in the 2015 American Heart Association Guidelines Update for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care. The updated content in this guideline included five parts: dispatch-assisted cardiopulmonary resuscitation (CPR), bystander CPR, emergency medical services - delivered CRP, CRP for cardiac arrest, and chest compression - to - ventilation ratio. This review will interpret the 2017 AHA guidelines update in detail.

        • Exploration and practice of high-quality development in emergency medicine under the health-prioritized development strategy

          China’s “15th Five-Year Plan” explicitly proposes the implementation of a health-prioritized development strategy, aiming to comprehensively enhance emergency medical care and response capabilities. In this context, the development of emergency medicine will face new opportunities and challenges. In order to provide theoretical and practical references for the high-quality development of emergency medicine, this paper analyzes the shortcomings in the identification and early warning of critical illnesses, the timeliness of emergency care, and emergency response capabilities in the development of emergency medicine in China. It reviews current practices with cutting-edge technologies in innovating diagnostic and monitoring techniques and equipment, optimizing and integrating resources, promoting interdisciplinary collaboration, and leveraging the complementary advantages of traditional Chinese and Western medicine. Furthermore, it outlines future development directions.

        • Role of First-aid Fast Track in Triaging Earthquake Trauma Patients

          Objective To elucidate the effect of first-aid fast track in triaging earthquake trauma patients by studying the earthquake trauma patients staying in the Emergency Department of West China Hospital after “5?12 Wenchuan Earthquake” so as to accumulate experience in the treatment of disaster traumas. Methods A retrospective study was done on earthquake trauma patients staying in the Emergency Department of West China Hospital after “5?12 Wenchuan Earthquake” from 14:28 May 12, 2008 to 14:27 May 15, 2008. Differences in care given during the time period were analyzed. Results There were 536 earthquake trauma patients in West China Hospital within 72 hours after the earthquake. Twohundred and seven earthquake trauma patients staying in the Emergency Department had an average stay of 129 minutes during the 24 hours after the quake; 104 earthquake trauma patients staying in the Emergency Department had an average stay of 97 minutes in the second 24 hours; and 226 earthquake trauma patients staying in the Emergency Department had an average stay of 86 minutes in the third 24 hours. Each consecutive day showed shorter average stays. Conclusion The first-aid fast track not only guarantees earthquake trauma patients are saved as soon as possible but also shortens the time in the Emergency Department.

        • Analysis of factors associated with long-term poor prognosis of tuberculosis meningitis: a single-center retrospective multivariate analysis of 119 cases

          Objective To explore the predictive factors for long-term adverse prognosis in patients with tuberculosis meningitis. Methods We retrospectively analyzed the clinical data (general clinical data, laboratory test results, and imaging findings) of hospitalized cases of tuberculosis meningitis admitted to West China Hospital of Sichuan University from 00:00:00 on August 1st, 2011 to 23:59:59 on July 31st, 2012. We collected data of prognosis results after 6 years of illness by telephone follow-up, and quantified outcome measures by modified Rankin Scale (mRS) score (0–6 points). According to the mRS score, the cases obtaining 0 points≤mRS<3 points were divided into the good prognosis group and the cases obtaining 3≤mRS≤6 points were divided into the poor prognosis group, logistic regression analysis was executed to find the independent risk factors affecting long-term poor prognosis. Results A total of 119 cases were included, including 63 males and 56 females; the average age was (35±17) years. Among them, 53 patients had poor prognosis and 66 patients had good prognosis. After univariate analysis, the age (t=–3.812, P<0.001), systolic blood pressure at admission (t=–2.009, P=0.049), Glasgow Coma Scale score (t=3.987, P<0.001), Medical Research Council (MRC) staging system (Z=–4.854, P<0.001), headache (χ2=4.101, P=0.043), alter consciousness (χ2=10.621, P=0.001), cognitive dysfunction (χ2=4.075, P=0.044), cranial nerve palsy (χ2=5.853, P=0.016), peripheral nerve dysfunction (χ2=14.925, P<0.001), meningeal irritation (χ2=7.174, P=0.007), serum potassium (t=3.080, P=0.003), cerebrospinal fluid protein content (Z=–2.568, P=0.010), cerebrospinal fluid chlorine (t=2.543, P=0.012), hydrocephalus (χ2=11.766, P=0.001), and cerebral infarction (χ2=6.539, P=0.012) were associated with long-term poor prognosis of tuberculosis meningitis. Multivariate analysis showed that age [odds ratio (OR)=1.061, 95% confidence interval (CI) (1.027, 1.096), P<0.001], peripheral nerve dysfunction [OR=3.537, 95%CI (1.070, 11.697), P=0.038], MRC Stage Ⅱ[OR=9.317, 95%CI (1.692, 51.303), P=0.010], MRC Stage Ⅲ [OR=43.953, 95%CI (3.996, 483.398), P=0.002] were the independent risk factors for long-term poor prognosis of tuberculosis meningitis. Hydrocephalus [OR=2.826, 95%CI (0.999, 8.200), P=0.050] might be an independent risk factor for long-term poor prognosis of tuberculosis meningitis. Conclusions Age, MRC staging system (Stage Ⅱ, Stage Ⅲ) and peripheral neurological dysfunction are chronic poor-prognostic independent risk factors for tuberculosis meningitis. Hydrocephalus may be associated with long-term adverse prognosis of tuberculosis meningitis

        • Research on Efficiency of H2 Receptor Antagonists and Proton Pump Inhibitors in Alleviating Gastric Mucosal Lesions Caused by Acute Alcohol Intoxication

          目的 探討H2受體拮抗劑和質子泵抑制劑(PPI)緩解急性胃黏膜損傷的時效性研究。 方法 對2008年1月-2010年1月在急診科就診的98例急性乙醇中毒后胃黏膜損傷患者,隨機分為對照組50例,治療組48例。常規給予休息、保暖,補液,維持水、電解質、酸堿平衡,維持循環功能等治療基礎上,對照組給予H2受體拮抗劑治療,治療組給予PPI治療。通過觀察急性胃黏膜損傷患者上消化道癥狀及體征,記錄不同飲酒及飲酒量,并根據患者就診時間及不同飲酒組治療后上消化道癥狀完全緩解時間進行比較。 結果 治療組上消化道癥狀緩解所需時間與對照組比較差異有統計學意義(P<0.001),不同飲酒組上消化道癥狀緩解時間上差異有統計學意義(P=0.000)。 結論 PPI在緩解急性乙醇中毒所致胃黏膜損傷的時效上更明顯,具有臨床價值。

        • Correlation between gender and long-term prognosis of patients with type B acute aortic dissection after endovascular therapy

          Objective To explore the correlation between gender and long-term prognosis of patients with type-B acute aortic dissection (AAD) after endovascular therapy (EVT). Methods From January to December 2012, all patients with type-B AAD undergoing EVT were enrolled by retrospective and observational study. They were divided into male and female groups. Kaplan-Meier analysis was used to analyze the correlation between gender and the cumulative survival rate. Results A total of 131 tyep-B AAD patients who had undergone EVT were selected, including 97 males (74.0%), and 34 females (26.0%). The medium follow-up duration was 2.1 years. Smoking history, cholesterol, white blood cell count, hemoglobin, creatinine and uric acid of the patients in males were higher than those in females (P<0.05); while the difference in other indexes were not significant (P>0.05). The inhospital mortality of male patients was 10 (10.3%), and was 3 (8.8%) in female patients; there was no significant difference between the two groups (P=0.803). Kaplan-Meier analysis showed that there was no significant difference in cumulative survival rate between the two groups (84.5% vs. 82.4%; Logrank test χ2=0.023, P=0.880). Conclusion No correlation between gender and long-term prognosis in patients with type-B AAD after EVT is found.
